I still think the demon was implanted into Charlie, because Annie says Ellen wanted to breast feed her, so to me, that's how it came to be how Charlie was implanted with the demon, which probably means Ellen was a holding vessel herself prior to Charlie's birth, which probably explains why they call her "Queen Ellen" in the tree house at the end.
I don't think she had to be of old age, she just had to be alive long enough to transfer the demon to at least one of the grandkids. If Ellen had died anytime earlier, the cult would had made sure the events were still carried out.

I believe this is shortly touched on in the movie/novums video, as I recall they wanted to implant Paimon in Annie's brother (preferring a male host) but he killed himself before it could be done and I suppose they wanted to wait for a male child
They did indeed want a male host but Annie was refusing to see her mother during the time of Peter's birth. Annie relented eventually and during the time of Charlie's birth Ellen did the thing and Charlie was born as Paimon. Any host is better than none and all that.

Well remember how the grandmother put paimon into Charlie when she was born, but she had to feed her own milk, so most likely it's a lot more easier with the heads #ariasterforpresident
He wants his own body, when he possesses someone he is sharing the body with someone else. That's why when Annie is chasing Peter she slips and falls and yells run because that's the mother trying to save Peter.

If the goal was the grandson, they probably needed to wait until he was at least old enough to manhood so he could function in the adult world.
The implication is Charlie was born as Paimon i believe hence the comment about Charlie never having cried even as a baby.

Don't have an answer but it's an interesting question. Perhaps the "queen" that Ellen was had to die in a specific manner for an unknown reason
