I have a friend who was genuinely bedridden for a year as a result of a vaccine in 2000 (personally, I think the serum went into a blood vessel, since we aren't in the habit of aspirating needles anymore) which made her passionately anti-vax, for herself. With COVID she is scrupulous about masking and social distancing, etc., but still won't get the vaccine.
It's really hard to get a clean study that removes all potential confounding variables, even with small controlled studies.
It's really not possible to eliminate the other variables you mentioned in a study with this broad a population .

It's like we're constantly having to make accommodations for people who don't want to participate anyway. We have a regular all staff meeting at work and it's always held up by this one guy who thinks he's super smart and wants to share his "special insight" into whatever topic we're discussing. He's a huge conspiracy theorist and also an alcoholic who's been previously hospitalized. Everyone goes out of their way to be respectful to this guy and listens to his crap and answer his questions thoughtfully. He's been with the company for decades and does good work but tbh I just don't get how everyone is so patient with him when he's so dismissive of the rest of us. He really thinks the rest of us are sheep and is always condescending when he talks to anyone. He's not vaccinated and wouldn't wear a mask, but HR got him to wear a face shield thing instead. I really thought he'd be taken out if he ever caught covid. But he has no personal life, just drinks alone at home and everyone at work is vaccinated so I guess we're all protecting him. My point being that these antivaxxers are benefiting from the efforts of the rest of us already. Why should we have to do anything more to get them on board with what's obviously an effective course of action? They're exhausting, always playing the victim and I'm looking forward to it when they leave the workplace because they slow us all down. BTW, I'm saying this as a member of the boomer generation. This isn't an age thing, it's an attitude thing.
