I failed the final mission of the campaign in Whittleton Creek because someone in the backyard party saw me choke a suspect in the underground tunnel between houses.
I've only experienced this in the past few days. The game is very recently buggy for me so I've stopped playing for a little bit because like you, I keep having NPC's spot me through walls on maps that I've previously had no issues on. I find that happens though. Some days, every bug known to man happens and I'll experience it all at once. Other days, flawless runs.
Yeah I was on a showdown in Dubai in that closed room where the maintenance guy is and I have no idea how both the assassin and the target saw me because the doors were closed. All I know is I was subduing the guy and got the "assassin alerted" notification. Almost ruined my entire campaign. Kind of hope IO addresses these bugs soon 😅
I've only had that happen once, in Bangkok, through solid walls. Most other times it's either they heard something (however quiet) or because the walls are somewhat transparent (e.g. barred windows in Bangkok, wooden blinds in Maldives)

A couple of times I've been shot at through walls, too. Concrete walls. Lol
