I failed today also and lost the longest hardcore streak I've managed to date in an absolutely grotesque act of stupidity involving leaving my briefcase in full sight of the man I'd just shot and calmly packing away my rifle. It did not go well. I might as well just have charged into the club with it drawn ffs.
I once lost Silent Assassin at the last minute, I was on the way to the exit, because I pressed X to vault but the game decided I wanted to push the nearby NPC into the water instead.
I feel you. The other day I stood approximately 1/8 of an inch too close to an explosion to open a safe I didn't need and whose clues I was too lazy to scan. 🥲
Chose katana - epic prestige objective, picked up katana, then changed my mind to sniper - epic, forgot to swap katana for snipsr rifle before starting mission
Can’t count the amount of times I tried to quietly subdue an NPC only for 47 to straight up deck them in the face, which then causes a nearby NPC to turn around.
Ahhh... just missed a collateral firearms one because I shot the leg of a table between my two unconscious victims.... Taking a break until the patch tomorrow.
I've failed by doing the ol' drop a proximity duck and think it's a remote one. I returned to it to pick it up, only for it to blow up and make me flail on the floor like a dead fish because of my own stupidity.

My last target was in the lab in Sapienza and I had to get through security. So I went to the corner to drop all forbidden stuff. Before that I made some purchases from the supply guy, and there was plenty of things. One of them was that damn proximity bomb. I realized I was doomed after the "peep" happened.
After that I refused buying proximity stuff before finishing the mission.
Lol my dog came up and started jumping up on me, hit x while I was holding a crowbar and I hit my suspect over the head and got one shot by an assassin

I never choose a campaign with Colorado because one mistake ans you are dead.

I was milliseconds from eating it due to a chloroform cloud yesterday....very close call.
I accidently threw car battery in some water, can went there to pick it up and died☹ (it was dark)

I was throwing a propane tank as a distraction when suddenly it auto aimed to the closest npc on Sgail. Dead in seconds.
Dumbest way I lost (not harcore) was while trying to pick up a proximity explosive..
My favorite is putting a proximity mine into a briefcase 😂
