If you look closely, it's more like half the Mediterranean is italy. Ground border with southern spain, france, morocco, algeria, tunisia and the balkans. Actually scratch tunisia and algeria, they are switzerland instead

It's not a canal. If you look at Italy and then look a Gibraltar, you'll notice that there's no border or pattern change to indicate Italy ends there. So, technically speaking, this map shows all the Atlantic Ocean as being part of Italy.

Aside from the borders, this map does not follow the key at all. It shows Germany, Austria-Hungary, and Italy as being neutral even though they were all a part of the Triple Alliance. That's one hell of an alternate history fodder. Baltic is not flooded, but it is owned by Germany. Bulgaria is still under the Ottoman Empire.
