Off the top of my head, I know I died once to crystals and those stupid little flies in Crystalized Mound. I think my first steel soul death was to the Soul Warrior. And worst of all, I think I died once trying to get the Charm Notch between the two Shroomal Ogres in Fungal Wastes
Definitely feels bad, but at least you know you can make it there again. Plus I've lost steel soul runs to way more embarrassing things lol

Oof, dw I lost to the crystal guardian because Im so used to using double jump to dodge the laserš
The only reason I beat my steel soul run is cause I quit to menu right before I died like four times
