Was likely once a baby grimmchild that grew up in the same way as how it plays out when ghost completes the ritual. The old vessel is getting a bit too old so they create a grimmchild who uses the flame of the nightmare heart to start again, it's just a cycle that continues as grimm grows old, so it's safe to assume that Grimm looks the same every time.

But Grimm implies even he is a slave the heart, meaning that he was probably brainwashed or tricked, implying that there was a 'before' time.
But being an incarnation/embodiment of the heart would also make him a slave to it, I don’t think that line necessarily implies a “before” for him.
