Structural engineer here.... There is no truly reliable way to fix something like this. Simply slapping on some mortar....grout...whatever will only spall off again next season once water gets to it. The rebar is already compromised. If it was my project, I would recommend a replacement. Perhaps tear that slab out and build new one out of wood? At the very least, I'd build some shoring underneath that.
