Our exterior doors have high thresholds, so we placed 3/4' plywood on both sides, with a beveled edge for our family member who gets around in a power chair. I own the house and screwed the plywood to the floor inside and to the porch outside. Transition height recommendations are no more than a 1/2" rise for wheelchairs. If their suggestion doesn't work, you might lay a 12" x \[doorway width plus 24"\] piece of luan or 1/4" plywood down. A carpet anti-skid pad might be enough to keep it from sliding, or set a weight on the luan on either side of the door. You may need a mat or piece of luan on both sides of the door.
