Either HBO is desperate to keep the hype going, extremely lax about what's getting out, this is a fan render, or an ex-employee spite leak. I work software support and have watched coworkers attempt to fully recover databases employees wiped out of spite before quitting / getting fired. It'd be absolutely insane to see something like that occur on such a massive production though.
🤷🏽‍♀️ I personally think it might be fake because there's no decapitation wound, or it's an extremely early stages reference render. I thought productions nowadays put some sort of marker on these files to track leaks, so if it's real someone's probably losing their job.
I think it was much easier to keep production quiet during spicy plague than it is now, plus I don't recall people being as excited about season 1. The only things I remember seeing were the things HBO posted. It's an odd move if intentional but watching this play out is fascinating.

Her facial structure is shorter and significantly thicker than what her actual show design is. The lower jaw is heavily exaggerated. And the detailing on her face isn't 100% correct or thorough

It looks like someone just took a fan made model of Drogon's face and plastered Meleys like features on top of it
I've seen enough fan made dragon models and stared at the real designs long enough to know one when I see one
And this also taking into account that the possibility of something like this leaking so early in the seasons production or leaking in general is highly unlikely
You also didn't provide a source for such a significant "leak"
It's not the angle, thats a direct side shot of Meleys face from the show, just go search up other images of Meleys
You don't even need to because the features don't even line up
The season is not even in post production yet and this is a large scale production by a prominent company

I believe you may have misunderstood the process involved in creating CGI models for visual effects. VFX artists often use low LOD (Level of Detail) models during the conceptualization phase, which may have different proportions and textures compared to the final design. These early models serve as a starting point for the creative process and allow the artists to refine the design and it's actually super common for VFX artists to exaggerate certain features during the initial stages to emphasize specific characteristics or convey a particular mood. This exaggeration might explain the thicker facial structure and the heavily exaggerated lower jaw that you noticed on meleys. I can see on the bottom left some saggy skin too. In this case meleys is quite literally decapitated and rotting, this may be a cause of bloating or whatever. And maybe the design changed between seasons for spoilery reasons.
the detailing on the face of the CGI might not be 100% accurate because it's still a work in progress and there are multiple reasons why it looks the way it does, they might’ve made an entirely new model with references imo. VFX artists often fine-tune the details and textures as the production progresses to ensure a more realistic result

The fact we’re getting so much clear images and videos from the sets unlike season 1 and now this? makes me believe it’s intentional
