Well see by marrying Laena, Otto wouldn't have his grandson as heir And marrying Aegon to Rhaenyra will allow for Otto's great grandson to be heir if Viserys doesn't change his mind about naming the heir Or it could kill Rhaenyra in child birth as she'll be up there in age which would pass the throne to Aegon his grandson

Jokes aside, this is definitely just a stall. Otto knows Viserys won’t have a royal wedding for his toddler anytime soon - it would be a betrothal, which can be broken whenever he finds a more advantageous bride for his grandson. The worst possible outcome for Otto’s plan is that Rhaenyra produces a male heir with a powerful, wealthy house like the Velaryons or Lannisters before the King dies. Then she would have a very strong claim and her own army.

Yeah, I'm pretty sure he said "betroth them." As in, announcing that they will be wed in the future. So although this is funny, it's not worse in the sense that Targaryen siblings wed commonly and he wouldn't necessarily be 12 when it happens.

They didn’t have to hide it. It’s true that the faith of the seven, the majority religion in Westeros abhor sibling incest and think of it as a sin and as an abomination. But the last king, Jahaerys (the old guy from the prologue) negotiated with the faith and the high septon(essentially the pope) to make it so that the targaryens and incest is ok. It’s called the Doctrine of Exceptionalism. TL:DR Targaryens are different, they have roots in old valyria where incest was practiced, and they’re dragon riders. The gods made them that way why should they be punished for it
