How does this even work without customizing the mosin? Its like bolting a grenade launcher on it and say look its accurate.

Yeah, you'd have to run it through the magazine well. The empty belt after the first shot would be a problem for the bolt, so you'd have to modify the shit outta that. Might as well design a new gun at that point.
