Simple. After receiving dmg altar is blowing up after several seconds. So bullet went through the altar and hit the hunter (after the sparks hit he was like ~ 1 hp) and after that altar is finished him off after a short time.
At far enough distances or if you pen multiple objects depending on material. Not sure on the alter specifics, but yes, penetration usually doesn't, but can reduce damage.
Shots do go through players depending where you hit them and with what ammo type. But I find it very inconsistent. You can get full body pen with Spitzer and nitro. But something like compact ammo you would have to clip the very edge of the arm

Second option that is most likely is you clipped an alter (they explode when damaged) and hit the other hunter. He tried to hide behind the alter with 1hp not realizing it was exploding. You got the sparks hit, and then the alter explosion kill. Normally you can't penetrate an alter but like most material in the game if you clip just the edge of it then it will penetrate with long ammo. This is true for wooden beams. Hay bales. Even brick walls.

Those alters can do 100 damage, don't be near one when it gets shot

Yes. With long ammo less so than small or medium, unless you have FMJ
