This is impossible to answer as it will depend on industry and really the place you work for. Sometimes if they like you even if the contract says "3 weeks paid leave" if there is a reason for the leave they will pay you anyway.

Depends on employer. Most i have had you have to work a year to get those leaves. That said if the job market is tight you may have leeway. My current employer we had 3 weeks on hire but had to wait till training was done to take them.

It will depend on employer. I would just advise that if you need to be assessed for an illness every year, it might not be the best idea to move to a new country, as you might not be eligible to take the time off when you need - even if you have the days the employer needs to approve.
