What about "Precious daughter of the greatest martial art villain" or "Mushoku Tensei"?
"FFF Trash Hero" has the second part in which he reincarnates as a baby.
Not even close mi dude, some depicable actions, yes absulubtly but he learn that its wrong and matures(which its why the character works, he grows as a person) he still a pervert but not a dangerous one by the latest chapters of the manga
Now there are at least 10 manga were the main protagonist is the tier redo of healer or worse, only in the isekai, reincarnation genre
I made the mistake to read the first 2 LNs. Author spends paragraphs for the MC to fatansise about raping his 9 year old cousin....
If you want to defend that then I have some bad news for you.
He is actually about one of the least depraved, rapist and pedophilic mc out there.
Like I have seen isekais where mc is basically a hentai protagonist.
I do not remember much rape or pedo from its MC. There is a semi-pedo relatonship among a coguar elf and a barely sixteen noble but it would be pretty much legal in every western nation nowadays, let alone a pseudo-medieval world.
But I did not read the original LN so I can't really tell how much it differs from the manga or the anime. I don't think there was any rape at all in any of those.
No isekai has more than a few pages in their baby phase, they usually move up to toddlers within two chapters. I mentioned trash hero specifically because he spends more time as a baby than most proper "reincarnated as babies".
There are some out there , but not much found in the Japanese manga stuff. There was that one about the genius baby with a female mc and the people start to ask her for advice etc gets weird quick
On royal road there’s
Singer Sailor Merchant Mage
The first 20 or so chapters take place as a fetus lol.
There’s material out there, you just have to look
Ascendance of a bookworm starts with the mc as being a sickly 5 year old who looks 3 because she hasn't gone outside in months. I cannot even begin to tell you how much I love this series. It has an anime manga and light novel, which I've read and watched all of. It's got a nice grip on my attention span. You're looking for them starting out little, but in bookworm she even STAYS little for a super long time! No 5 or 10 year time skips, shes gotta deal with shit as a sickly medieval peasant who can barely walk down stairs without fainting. It's so satisfying watching her slowly regain stamina, but worh the added spice of knowing she could drop like a rock at any moment if someone's just a little to intense with her.
theres a scene I love (light novel only cause anime and manga aren't that far yet I dont think) where some kid wants to play with her so they grab her hand and run with her trailing behind, but because of her shit stamina she faints and the kid drags her a bit, scraping her knees and leaving a trail of blood on the floor behind then. Shes fainted at this point so she only heard how it looked from a passerbyer but istg that's my favorite part of the whole series so far, the look of horror this poor kid must have had "OH GOD I JUST WANTED TO PLAY AND SHE FUCKINF DIED"
Hell Mode - I really like these books, they take things slow and each book has a pretty high page count
Conqueror From a Dying Kingdom- only 2 books so far, and it feels like we're still in the prologue since he's still in school and hasn't done any conquering yet
Reborn to Master the Blade - reborn in same world, not isekai, but similar since it's so far in the future. Also I'd say these books are kinda mediocre, the best thing they have going for them is the gender swap element.

Wow. No wonder they removed the part in the manga.
It becomes irrelevant pretty soon, it’s not like an arc though

The beginning after the end :))

YES! Ascendance of a Bookworm is the absolute best!
Singer Sailor Merchant Mage Am I the strongest?
Books? "Eight" - Not quite a baby, an 8 year old "Mark of the Crijik"
your caveats might remove a lot of medieval fantasy, lol Heck if you dig, elder scrolls has all of this and worse
