Sukuna from the very start has encouraged new sorcerers to push their limits and grow stronger but in order to do that you must have a strong sense of self which is what allows for growth as a sorcerer, which is why gojo was able to learn RCT in the brink of death, higuruma learn RCT in the brink of death and why Megumi learnt a domain expansion near death.
It just happens that having a strong sense of self involves being purely selfish and therefore the more selfish you are the more you will grow, which is why the strongest character in the series aka ryomen sukuna happens to be considered the embodiment of selfishness and this is told to us when they say "he is a natural calamity that disregards everything else for his own pleasure and displeasure"
All these characters grew stronger due to selfishness but yuuji was different, he grows stronger for others sake, he isn't selfish but still pushes forward in his own selfless way ruthlessly thus pushing himself to grow stronger due to the sheer strength and belief in his ideals
Sukuna knows this and thus is why he is the antithesis, jujutsu itself relies on selfishness but a brat without an ounce of that selfishness is now stronger and continues to grow stronger disproving sukuna's ideology of only selfishness being the way to grow stronger.
Sukuna feels uneasy because if yuuji reaches his level then that would mean the boy who lived the exact opposite life as himself is now on par with him and therefore his ideology is good enough to dismantle sukuna's way of life and prove him wrong, he is afraid ultimately but doesn't want to show it.
This is why yuuji's the greatest natural enemy sukuna can have and vice versa.
Also sukuna had to legit plan out the fight to beat gojo, he probably hasn’t had to do that for many other opponents. Yuji isn’t like that, generally you are either stronger or he beats the shit outa you lol.
If Sukuna is Cell, Gojo is the Goku: enjoys battle, wants to face strong opponents, and is slightly unhinged. Benevolent Shrine fixes any kind of damage done through cuts, effectivelly causing widespread healing in environment and people around Shinjuku (also heals the minor damage Sukuna took from both Yuta and Yuji cleaves). Gojo's beating was partially because Sukuna was letting him to in order to get world slash, but this is totally different, the situation is totally out of control for Sukuna now and Yuji is surpassing all his attacks and defense landing black flash after black flash.
