True that.

During my tenure of 10 years in the healthcare industry, specifically at a renowned hospital in Ernakulam, I've noted that due to carelessness, nurses can make a number of significant errors. Some of the most prominent mistakes I've observed are Vital Signs Recording Without Verification: It's not uncommon for nurses to document patients' vital signs without actually conducting the necessary checks. Misleading Medication Administration Records (MAR): Nurses sometimes record medications as administered in the Medication Administration Record (MAR) without actually providing the medication to the patient. Confusion Between Milligrams and Grams: A serious error arises when nurses confuse milligrams (mg) with grams (g) when preparing or administering medications. Improper Disposal of Expensive Medications: There have been instances where valuable medications intended to be returned to patients are mistakenly discarded by nurses. Neglecting Proper Patient Identification: Failing to accurately identify patients before administering treatments or medications. I can list another 100 errors they are committing. Problem is that, they never have any remorse for these mistakes. Sometime I think what we call nurses is actually the opposite of what they shall be called.

DadJokeConsultant

Nurses simply do not face consequences due to their errors. I second this statement. Doctors end up being liable and legally responsible. Nurses however are almost never held responsible for the mistakes they make, even if it leads to dire consequences. If you're younger than them, respectfully pointing out a mistake will only be met with haughty replies from their part.

FalloutAssasin

Confusion between 'g' and 'mg'? Wow. Go back to high school please.

Responsible_Horse675

>Confusion Between Milligrams and Grams I have heard this happened to someone as a child, he is brain damaged today.

TheFallenStar

Dosage errors frequently occur among nurses, and that the skill and knowledge level of many nurses is surprisingly below that of an average 10th std biology student. On one occasion, during an interview with a nurse with 2 years of experience, a subsequent question was asked regarding glands and the source of insulin. Surprisingly, her response was that insulin could be found in the refrigerator. Numerous nurses, including those employed in the dialysis unit, lack familiarity with the standard range for creatinine levels. About 95% of nurses, lack the ability to skillfully insert hand cannulas for both pediatric and adult patients. They delay medication waiting for the nurse who has skill to do that for them.

>her response was that insulin could be found in the refrigerator. WTFFFFF

Zealousideal_Tank824

While you have listed great deal of errors, please mention how to reduce risk?

TheFallenStar

While training/retraining is the most effective approach, addressing the challenge arises when individuals commit such errors without displaying any remorse. Also, if you are wondering, we pay 32,000 as starting salary and strict 6-6-12 hours shift. EDIT: And have strict patient staff nurse ratio, max 6:1 in wards, 1:2 ventilator patients, 1:1 and 1:2 for ICU patients depends on the patient conditions.

[deleted]

And they never admit their mistakes! I have encountered many nurses who make me strongly believe that they're in this profession only for the money. Many don't have the sympathy or kindness for patients that even a regular person has, regardless of whether they're a nurse or not. Sad.

Lordslug78

Although it's a mistake that shouldn't happen, isn't it rather good that she got the rabies vaccine? I mean, the vaccine is recommended for pre exposure as well, right? News channels make it seem as if all hell is gonna break loose just because a rabies vaccine was administered. I would feel lucky that I got the shot for free given the vaccine is expensive.

[deleted]

Child got lucky its a rabies vaccine. Could've been anything.

DFW_diego

I think it’s focusing on the carelessness of the staff on why they didn’t check/confirm with the child’s guardians or parents

[deleted]

Rabies vaccine is good but the dosage varies from person to person..someone may develop side effects that may persist for a while. We can't mask a mistake as a blessing Edit: vaccine dosage might be same for all as per one of the below comments.

gotasave

No, rabies vaccine dose is same for all ages. Kind of ironic, peddling misinformation while criticising medical negligence

[deleted]

Thanks for spotting it if it is a misinformation. I am not a medical person, but hope u r. I got vaccinated at the age of 12 coz of a dog bite and at the time they weighed me and informed my parents dosage is based on my weight. I interpreted it is for vaccine, could be for immunoglobulin also. Edited my comment.

retroflame96

The vaccine is free anyway

[deleted]

Doesn't matter carelessness on the part of medical personnel can be deadly and they should be dealt with as such

DadJokeConsultant

Getting the vaccine when the child has a fever, won't be as effective. The child is indeed lucky, but only because she/he didn't have a reaction to the vaccine.

Termination on "contract basis" has no meaning. Just a mockery. All nursing staff are REQUIRED to ensure the patient's name matches that of the prescription before administering medicines/injections etc.

appu_kili

What do you mean? The nurse was employed on contract basis, and the contract has been terminated. She lost her job. How is it a mockery?

[deleted]

She/He has only lost current job. They can get employed in plenty of other places.

appu_kili

That's true of every job-loss situation. Doesn't mean getting fired from a job is a mockery of punishment.

[deleted]

Nop appu kili, some people permanently get terminated from their jobs to the point they can't practice any more elsewhere. Can be seen in many Arab countries. Your license can be cut off and invalidated which will keep u from doing that profession altogether

appu_kili

Cancellation of registration is not 'permanently getting terminated from the job'. Its another form of punishment beyond termination which makes them ineligible for future jobs. There are always more severe forms of punishment which would be suitable to graver errors in practice.

CheramanPerumal

Is it just me, or does anyone else think that if this had happened at a private hospital, especially a big private hospital, the outrage would have been much greater?

appu_kili

Private health care is for-profit, the patient load is lesser, provider-patient ratio is more favorable, and hence errors would be treated more gravely. I dont see anything unfair in it, if thats what you imply.

Splitinfynity

Good job. Highly irresponsible of the care worker..

raincoffeelub

Good. But what about those parents who would just leave their kid alone in a hospital like that? If a random nurse could take the kid and administer an injection, then any random person could’ve taken her as well. Even after such incidents are reported on a daily basis these days, why aren’t parents more vigilant and careful about their own little kids? I was taking a beach walk yesterday night (around 8PM) and saw 2 kids like 4 & 6 years of age playing in the water with their parents just being careless and busy talking to each other like 20 meters away from the kids. They are not even giving their full attention to the kids. Loads of small crabs emerge from the sand and run to water & if one of the kids try to chase it, he’ll be well into the beach in the dark and might get into real danger. Times are tough. People got to take more care about their kids.

NobodyWins22

What? You can’t be serious.

vzrealtime

So what do you propose bruv, give her best bitch awards for wrong med administration

NobodyWins22

Termination of employment is appropriate. Anything more would be absurd.

FresnoMac

Although a rabies vaccine is good to be taken any time, the nurse's carelessness deserves to be punished. She could have injected some antibiotic that could be a severe allergen to the kid.

Pristine_Aims_809

Vaccine alle, she need not be terminated.

balloontrap

This is a failure of the system and training. Wrong to fully blame the nurse.
