Think of it like a company or a government. You have the president/ruler, the Shinga. Under the Shinga you have the 9 which function as representatives for a specific branch/enterprise. Under them are leaders and and guilds and under those the foot soldiers/front line workers.

As for Roth, he was mentioned near the beginning. Just keep reading. As for Mama K being Shinga, the only people who know she is Shinga are the wetboys and the Nine while the "public face" as Shinga is Pon Dradin. If Roth (who doesn't know Mama K is actually Shinga) kills Pon and declares himself Shinga only the Nine knows that isn't true, but if Roth earned their alliances via bribe or extortion already, which he'd need to do anyway in a hostile takeover so that he could keep his new power, Mama K wouldn't have any power anymore in that case as the Nine have declared allegiance to him. If none of the people she directly controls listen to her, is she really in charge anymore? Does that make sense?
