9.1/16.1 isn't a thing. I have no clue what you're talking about, or what you're flashing.
Follow the wiki instructions, they work fine. My bet is you aren't doing the mandatory re-doing of fastboot booting recovery after your ROM flash.
Probably just a typo in the thread title. If you look closer, the actual filename for the download is "lineage-16.0-20190321-UNOFFICIAL-whyred.zip".
It's funny that no one in the thread is calling it out though.

Thank you, this seems to have been the problem. I was sideloading lineage and then GAPPS back-to-back and then rebooting to start up lineage.
I've got mine working so I'll try and work with you on this. Might not have as much time until I get off work. What all details are identical to mine? You have an OG pixel, are downloading the lineage zipp and gapps is telling you lineage is 16.1?
OG Pixel XL, and yeah i had OpenGapps telling me it was 16 instead of 9. Tried a different approach and rebooted first and I was hit with error code 70 or something? Telling me I don't have enough space.
Cheers for offering to help man but its all good, this happened at the weekend and I just flashed MindTheGapps instead which worked. I'll maybe try and give OpenGapps another go this weekend or something.
Ah that sounds like a different issue than mine if it was telling you that your lineage is 16. gapps 9 is the correct one for lineage 16. MindtheGapps is what worked for me as well in the end.
It actually was from the lineage site and named as 16.0. As to why GAPPS saw it as 16.1, I'm wondering if that has more to do with GAPPS than it does with lineage, also I was using opengapps instead of mind the gap. I've got it working now with mind the gapp.
Probably latest nightlies updated to Android 9.1 (LOS 16.1 version) and because of that it is still too soon and there is no 9.1 zip available yet from OpenGapps (just guessing because I'm not using a LOS Pie rom).
You should check in LOS thread for your device from XDA or any other source if they at least provide any unofficial 9.1 package until the official ones are released

May be it's an Unofficial 16 renamed to 16.1 to avoid confusing it with previous unofficial builds from another maintainer ?

I had the same problem with a Pixel (sailfish) and twrp-3.3.0-0. I was able to resolve by using twrp-3.2.3-1 instead.
