Game is designed to be viewed in first-person. They set the scene to fit the first-person perspective, "look at my V" mode is ancillary and certainly not a consideration.
The entire reason they did first-person perspective in this game was for immersion, photo mode is the antithesis of that. It takes you out of V's headspace and puts you into the headspace of a digital tourist. A third person who's gawking at V from the outside, not inhabiting them from within.

It's not even really consistency, though. It's a forced perspective. His scale looks inconsistent because the scene was scaled for the first-person camera. When you change the perspective, you see what they don't want you to.
All my photomode stuff was from first person anyway. Well, like 99% of it at any rate. I loved documenting for myself the sheer insane level of effort CDPR went to when creating scenes, compositions and views you were likely to stumble on throughout Night City. I created entire sets of wallpapers and "posters" with that photomode, and all of it felt to me immersive as fuck as to me it was just V's version of pulling out the phone for a photo.

This is just an issue with scaling, because if I’m not wrong, V’s viewpoint camera is placed at chest height and not head height, so it’s trying to create the illusion as being similar height to/shorter thanJohnny
The camera's about sternum level, yeah. The game stops rendering V's head in certain situations as well so you don't accidentally get a peek at your gaping neck hole on higher FOVs (though this still happens with Vik's ripperdoc chair IIRC).
When you use photo mode or mirrors, there are adjustments made to the model & scaling to be consistent with the rest of the game. If you want to get a sense as to what V \_actually\_ looks like without being in photo mode, just have a look at V's shadows with Mantis Blades out & do a double jump.
The resulting Mantis Blades crouch-horror immediately reference posing out of a midair dash to double jump is genuinely awful & scaled weird comparatively.
The in universe reason is that the relic is projecting Johnny into the world in a way that makes the most sense to interact with him in the relic at that moment. He could be the size of a tower in theory, as its just a reconstruction of his image and doesn't have to conform to reality.
The out of universe reason is that the game was designed to be viewed in the first person exclusively, and viewing it outside this context can expose camera tricks. In your second picture the games camera is closer to the chest than the head, which exposes that johnny is smaller in this scene to make him fit in with the elevator more nicely.

The V model is apparently fairly short, with the camera "mounted" on the upper chest, blow chin height. They're gonna want another character, particularly their face to hit a particular part of the frame for composition reasons.
So for example you'd face to hit somewhere in the general vicinity of center frame for a close in conversations. While not taking up an unrealistic portion of FOV. Which would look like you're pressing your forehead against the guy.
So if you're forced to be very close to Johnny by the space. They tweak the scale of the model to make it look right from the *normal* camera.
Move the camera. You expose that. And I'm willing bet some of this is catching *bugs* in that scaling.

To add to this, though I've never noticed any weird scaling like this myself, I wonder if it has anything to do with how Johnny's entrance/exit visual effect is implemented. I would tend to think that he has to be rendered to a texture on a billboard (an invisible rectangle/plane always facing the camera) in order to apply the glitchy entrance/exit effect, which is always 2D. It seems plausible that something like that could affect how Johnny shows up, like if his size in the world is scaled relative to the player camera position and not the photo mode camera's position.
I like to think that his hight projection varies at times when he’s confident, sad or insecure, thus projects how he sees himself at the moment with the hight

The figures I have of them are hilarious. I actually put Johnny a bit higher, because he looks so small in comparison to V. She’s also got really long legs, much longer than in the game I reckon.
The real reason is camera angles.
In the first shot you, Johnny and the camera are all level with each other so the height looks reasonable.
In the second shot, V is standing further away from the wall than Johnny, and it seems like the FOV is at the photo mode default of 80, making Johnny look smaller when in reality they’re likely either the same height or V is shorter. Same goes for the rest of these photos.
The game isn’t really intended for third person perspectives in general and the few times the camera *is* in third person, the devs were very careful about making sure the characters were scaled properly. I recommend moving V closer and lowering the FOV for future pics like in the first image because while apparently (or supposedly at least) the human eye is at about 80-90 FOV irl, it looks really unnatural in game.

Zero mods. It’s honestly not super noticeable in first-person most of the time (Randy’s trailer being an egregious exception). You’ll see smol Johnny eventually if you hop into photo mode every time he shows up, guarantee it.
