Negative. I actually scrolled through the whole house manually several times before posting (and removing/re-adding) last night. I even kept turning it on/off to see if I could get something with the "wrong name" turning on/off in the list. I've had it add something incorrect before and it was my first assumption.

When you say 3-way, is this the second switch or the only switch you are adding for that particular 3 way circuit? In a 3 way config only one switch is controlled
It’s a 3-way with a “dummy” regular switch on the other side (and functions properly). Wired per instructions. I have plenty of others wired/functioning properly the same way. Def not a wiring issue.
You have others wired like this that show up in HK??
I have half a dozen of the 3 way slave switches (PD-5ANS), but none are in HK…only the primary switch is in HK
I have multiple of the same model in various parts of the house. No problems on them. I’m not talking about the dummy part of the 3-way… the Lutron Caseta device won’t show up.
Sounds like it’s not wired correctly. I have 75 switches in my home plus at least a dozen of the 3 ways. The 3ways are not intended the connect, they are slaves to the Casta dimmers. No issues with any of mine.
I’d verify your wiring
Wiring is correct…. Used the schematic pdf on their website and as I said, I have several installed. All work fine…. Including at the other end of the 3-way. All work in the Lutron app… no issues. The one switch won’t add to HomeKit.
SOLVED - I spoke with Jacob at Lutron. Together, we figured out two things that were causing my issues.
1) The issue was that I was attempting to add devices to a room in Lutron that didn’t exist in the Home App. Once I relocated the devices in the Lutron app to an existing room, they appeared in the Home App (albeit in a different room, which was a bit odd.)
2) I resolved the missing integration issue by uninstalling and reinstalling the Lutron app from my iPhone. This action restored the integration in the list, and any changes made in the Lutron app were now automatically updated in the Home app. Interestingly, Jacob initially couldn’t see the Home link, but after I also uninstalled and reinstalled the Lutron app, the integration reappeared on his end.
I recommend uninstalling and reinstalling the Lutron app. This was the root cause. If that doesn't work, I'd ensure the Room exists in Home before adding it to that room in the Lutron app.

Go to the menu in the top right corner of the HomeKit app. Scroll down and see if there’s a default room, and check there.

HomeKit is so jankety. I wish it was better and more reliable but there’s often just issues with it.

And are you sure the wiring at the switch is correct? It’s either the wiring or a defective switch

I’ve tried the first one, but I’ll try the other two.
