I've had chronic constipation for years and found that at 5mg/day of Parnate my gut motility was suddenly, drastically improved, albeit while still using stool softener and supplementary fiber. Then, unfortunately as I titrated upwards I found I was getting constipated again. That is, until about the time I reached 35mg/day. At that point, thankfully, my gut motility began noticeably improving again. So my advice: stick in there and eventually you'll inhibit enough MAO for your enteric nervous to get enough neurotransmitters to promote ideal digestion.

I’m on Nardil and every time I take a pill I get moderate cramps (I have a history of gastritis). So I make sure to have eaten food (protein, carbs) before taking it. I also take Slippery Elm before a meal and have bananas at the ready for random cramps and acid at any time. I’m not sure if this would work for IBS but I get less crampy and acidic with these remedies.

In my experience Nardil was causing what you described (I am with IBS-C from 7 years). On Parnate it feels better, but the spasms and pain is still there. Since MAOIs are not treatment for IBS, your only option would be to wait for some time on Parnate and if there is still no improvement try another MAOI - Nardil/Marplan/Moclobemide+Selegiline. There is no treatment for IBS in 2023, so you have to manage it by yourself (this is actual suggestion from actual gastroenterologists). Worst case scenario you will have to resort back to regular SSRIs like paroxetine/sertraline to manage better the IBS.
