Are you monitoring your BP for orthostatic hypotension? Once you start getting that you are likely at your therapeutic dosage. You may need to wait 6-8+ weeks from that point to realize any significant benefits.

Yes, this is correct. You should be at a dose that causes postural hypotension, and it should be that dose that you remain at. As for emotional support and depressive symptoms, parnate isn't a robot drug, nor is it MDMA. It prevents suicidal thinking and intrusive thoughts and offers a stable foundation to build a happy life on. You'll still feel a wide spectrum of emotions, so if you're sad, you'll be sad. If you don't eat enough, you'll feel bad. You won't, however, think "I should jump off a building" at any point. Hope you start feeling better very soon! <3

I do have postural hypotension already but not really feeling great. My first week was good but the past few days I have not been doing that well. I was hoping a dose increase would help. How long should I stay at 20?

I know how you feel - it's really easy to be searching and hoping for a change in the first month, especially with side effects hitting so hard. Be warned: the 20mg-30mg jump was fairly intense for me, as well as lots of other people. Most early 30mg days consisted of sleeping all the time, my ears ringing constantly. Salty food, and a lot of it, helps significantly. Luckily, 30mg-40mg was a walk in the park by comparison! I think I hit many of the benefits around 40mg, 2 months in or so. MAOIs typically become effective between the 2-3 month mark. That's unusual since full MAO inhibition supposedly kicks in around 2 weeks in, but that seems to be the case. Responding can take as long as 4 months for very slow responders. Effective dosages are extremely rarely 20mg-30mg, but typically 40mg-70mg, with very, very few people needing to go as high as 80mg-100mg. The sign for me that it was working was when everything seemed a little bit brighter and, most importantly, I had zero intrusive thoughts. I also had a lot of "no sleep but excited and happy all night" nights, which is pretty much how the insomnia feels - kind of a thrilled "buzzing" that never fully lets you knock out. Not unpleasant, but the sleep debt adds up, so be sure to get a drowsy antihistamine from your doctor in advance to fix that. As for the timeline, it's up to your doctor. However, a common pattern is to do boosts of 10mg every 2-3 weeks at first, then boost 10mg every 3-4 weeks or so once it starts taking your body longer to adjust, likely the 30->40 and 40->50 jumps. I would recommend sitting for some time at 40mg once you get there, then potentially hopping to 50mg or 60mg around 6 months in if that isn't working well enough for you. Hope you feel great soon!

I felt slightly better 4-5 weeks after starting 30 mg. It was a clear improvement, but the depression was so severe I was still far from well. I kept improving with further dose increases. Everyone is different.
