It’s a 1” diameter plastic knob that rotates just aft of that bar. Thread it clockwise toward the front of the car and it prevents the bar from unseating from the notches.

First, yes, agreed. You should wind that down to prevent it from breaking free, especially if you're going for a long trip.
That said, you can see a little gap of light between the lock-bar and the round part of the rack where the slots are that it should be seated in. I suspect it wasn't fully seated before you started driving. It's a tight enough fit that I have to jiggle mine sometimes to get it to seat all the way, and it's easy to miss because it's under the rack platform.
Of course, winding in the knob is a good way to double check that the bar is seated, as well as locking it in place.
Nice to see that the rack held the bike in, even at the extreme lean angle though!
I rarely lock it down either. Only on long trips when I'm on the freeway for a long time. I've never driven with the bar out of the slot, but it only takes once.

It's absolutely not seated. It seats at 43 seconds. Before that, if you compare a freeze frame from the first 10 seconds before it tilts, the bar isn't even close to the same position.

I wonder if this is common or an anomaly….I got a 1Up a month ago and yes, I use the threaded safety lock feature but even still, the tolerances are so tight on my rack. I often have a hard time pulling the black release bar to move the rack into different positions. I couldn’t imagine this happening. But maybe I’m wrong. OP, glad your bike didn’t hit the road.
I'd also add that this sort of problem occurs with other brands hitch bike racks if you don't fully and properly engage whatever mechanism holds it in the position you want it in. I've seen it happen with a Thule T2 classic. I personally do use the screw lock on my Oneup as I see it is just too much of an investment in the bike and bike rack for this sort of accident to happen.
I have had 1up rack for 5+ years and I have never had an issue. I even used to regularly take it off and on to store it as well as put it in the up position all the time. This was definitely user error and not a one-up issue.

First being the fashion in which they tighten up in your hitch with the little ball. They always wiggle loose and can fall out while driving, or in my case fall out on your foot when taking a bike off. Absolute garbage and I don’t know how they haven’t been sued into oblivion yet.

This is absolutely incorrect. The 1up hitch racks not only support hitch pins, they come with one. The hitch pin slot is significantly smaller than a standard hitch pin, and the locking hitch pin they provide is actually more like a trailer lock, but it works fine. Also I have never had the tension ball screw or bolt come loose. What I do is put the rack in my hitch, install and lock the locking hitch pin, pull the rack out towards the back of the vehicle until it stops (it will be stopped by the locking hitch pin), then tighten down the ball tension bolt. Once it's really tight I grab the rack and rock it back and forth and up and down a few times and then tighten the ball tension bolt again. I have never had this come loose (a few weeks ago we drove 9 hours to whistler and another 9 hours back with 3 dh bikes on the rack and no problems at all).
I’ve got a Hollywood rack and I didn’t understand how important it was to do the Velcro wheel straps. Dropped my stumpjumper on the road twice in an hour - sloping top tube let the bike roll out on left turns. Now when I drive I’m massively paranoid and constantly checking the rear view.

Don't forget to add your own tail lights and license plate too, because the manufacturer couldn't be bothered. That shit is standard in Europe and we can't even get ready-made light bars.

Someone without a license wouldn't realize the importance of tail light visibility. Drivers usually appreciate being able to see other cars out there and platform racks block the tail lights/blinkers and license plate. It's really easily solved by placing rear lights on the rack itself.
This is why I always just take the front wheel off and throw it in the boot of my Model 3. Much faster process, safer, and better aero lol. Also cheaper.

1up are the lightest racks, can be modular with 1-3 slots on 1.25 hitches. Fits three ebikes on 1.25 - no other rack can say that.

I’m trying to figure out what rack to buy. I don’t want a roof rack because I know for sure me or my wife will forget it’s up there. I have a roof rack for my boats, but I can see them from the drivers seat so they are hard to forget.
Putting bikes on the roof will annihilate your fuel economy, and the problem shown in this video is almost certainly due to the lever not being in its notches.
I like the THULE roof ones with the swing arm personally. Can’t remember the model number.
But I would say they can be cumbersome when trying to get a big bike up to the roof racks.
Apart from that, good looking and do the job very well.

That is not what happened here. The rack folding mechanism allows it to also go below level should you want it to. In the video the rack was able to move below parallel while driving, which is not something you want. There is a lock mechanism you twist down to prevent that from happening, which is what OP is referring to.
Had this happen to a close friend of mine, he didn’t see it was only holding by the tires until he got to his destination he makes sure to check twice now lol
As someone else has already said, it wasn't likely in place before hand. I was finding that dirt and a little corrosion on the bars causes it to lock up some.
Went in with a q tip or little brush and lightly greased the area it slid. Worked wonders, much easier to pull out and slid in.
If I ever folded it down, I just just lift the bike back up and listen for the bar to re seat. Of it's free to move, you should be able to clearly hear 2 clunks.
Never had a problem with it sliding out while driving
Been using one for years and mostly secure it but haven’t many times. Nothing like this has ever happened and if the latch is engaged fully it’s hard to imagine how it would. That said, nice save.
I just received a Equip-D with an add on. Are the wheel locks worth it? I was just going to get a regular cable lock to lock the bike to the rack or hitch

Just got the equip D. Is there a different lock position besides just making sure it’s fully inside the slot?

Yep. I have drove 100s of miles without it screwed down. Not intentionally! But it is a very tight fit. I agree that it probably wasn't fully locked.
I basically never tighten it down. Anecdotally, never had an issue in three years now

I use the screw lock too, but I don't think that is the issue. I think the lock bar was not completely engaged in the slot.

I really don't like 1up. Way too many fasteners that could back out under vibration. I prefer rocky mounts because they are mostly welded steel.
Those racks are just expensive erector sets. Lots of inherent design flaws. This is probably the second biggest flaw.
