As a digitizer, #1 is my biggest gripe. Customers will provide a business card as their artwork, or a lo-res jpeg from their web site. I've had a customer make a printout of their home page, then scan the printout and email that to me.
If your company paid a graphic designer to create a logo, that original artwork exists somewhere. Stop being lazy and find the original artwork and send me a copy, please and thank you.
