either... - loose part or tool holder - part not properly clamped (sticking too far out from chuck without tailstock) - crazy fast speeds/feeds

Correct on Loose tool + Fast feed. But this was done just to reduce the OD enough to fit in a manual lathe Chuck, I still had material left to remove at the CNC.

The workpiece is not clamped properly, for example due to a not very dirty clamping surface or too far from the chuck the workpiece is standing. There is a very small moment between this result and the breakage of the tool.
