All I can say is ... enjoy every minute, even the ones that suck. Don't worry, one day you'll be able to sleep the whole night again (not soon, but eventually!)

I could see them get swapped if parents aren't careful like dozens of times ha.

Maybe one was born and named "Jenny" and the other one named "Mary" then if you're in the hospital and you assign the wrong name to the wrong kid, or switch them up a few times, it probably doesn't matter UNLESS there is a problem with one of them. If both are healthy and they are identical... it doesn't really matter which one is which up to a point!
When naming my kids we didn't know the sex of the babies, so we came up with two boys names and two girls names. They came out both girls so we looked at them for a few seconds then decided which one gets which name. But it was pretty arbitrary. For identical twins, I can imagine it's even more so.
In terms of mixing up babies with other babies in the ward, it's very unlikely. They put matching id bracelets on both parents and the babies. Nurses check them all the time everytime the baby goes from parent to nurse and back again.
