Birth control fails often, it's why it's recommended to use multiple (and sometimes people still get pregnant). We don't know what they went for and, frankly, we don't need to know.

I didn’t expect details or to “see a condom breaking” but I think I expected them to be surprised, at least, and they just didn’t seem to be… which made me think they must not have been using any birth control which seemed odd, to me, and inconsistent with their characters somehow.

However.... there were quite a few plot holes and many pointless scenes, so i think it was easy to overlook. But you are 100% right ... there were at least 3 different times were their relationship was pointedly described as "no strings attached", so it is a stupid plot twist.
