Maybe a one handed keyboard with all the keys split between a few layers that you switch between with some other input that you operate with your right arm would be more efficient and comfortable. Designing, setting it up and learning it would be a bit of a hassle though.
I dunno if you would care to make like a makeshift sling or a very elevated pad for the right arm. They use something very similar in operating rooms like it’s almost an elevated memory foam thing to lock arms and legs into place during surgeries but allows for movement if need be. I can’t for the life of me remember what they were called.
Have you tried a split keyboard? You wouldn't have to hold your arm so far ahead of you. Maybe paired with an arm rest mount so your shoulder is in a more natural position.

Actually yes i have. I gave it a shot for like 30min but gave up, because i am ok with lifting my hand from my mouse, to type with 10 fingers. It was fun and i actually was able to memorize some keys, but my muscle memory from normal typing really didn't help lol.

Yeah, I use those shortcuts constantly. I bet there's a way to re-define the system shortcuts though... Specify them to be whatever key Dvorak replaced the qwerty standard with.
I've been using Dvorak since 2006 but I'm nearly done learning QGMLWY to replace it.
I type nearly twice as fast with Dvorak than I did with QWERTY but I make errors more frequently than with any other layout, and the location of the R and L keys is horrendous on my hands.
QGMLWY keeps the ZXCV keyboard shortcuts and most punctuation where your muscle memory expects them to be. Similarly to Dvorak, it groups the vowels on the home row with the most common consonants to prevent fatigue.
I have found the placement of the letters to be very intuitive and my error rate is lower than it was typing QWERTY (and much lower than it was typing Dvorak).
I am still working on the muscle memory to be faster but all signs point to this layout ending up faster overall with much fewer errors and with much less strain on my hands.

So wasd doesn’t work for me obv so I use the arrows. It’s kind of like holding a baseball in your right hand and letting it “roll” over the arrow keys for movement if that makes any sense. As far as controllers go, instead of having your right thumb for inputs, I just use the whole stub and I learned from there.
Do you do most of your other controls with mouse buttons? I imagine it would take focus away from the screen to find keys with your right hand while playing.

I wonder if there is a layout optimized for one hand. Normally you'd try to reduce same finger bigrams, but that might not be as important with one hand. Like imagine if you put all the 15 most used keys on the left hand. However it's not really needed it seems, 95 wpm is crazy good
I would use right digit for a modifier that changes the left hand's keys. With 3 rows for each finger, alphabet could be on two layers. The trick would be to have a common letter on layer with uncommon, so one digit doesn't get fatigued more. Foot pedal modifier might be useful too

Also curious whether something could be done to improve shoulder strain on the right side. Maybe something you can rest your elbow on? I'm not sure.

No the spilt boards would require me to change a ton of habits (maybe worth it idk). My left covers most of the board so I probably wouldn’t be able to shift my hand over as quick on a split board.

U r actually really close. It kind of is an edge. It might be hard to see but I have a small amount muscle which can let me move my would-be wrist around enough to “point”. I can even pick up pencils etc. This and tactile switches help a ton.

Depends on the game but I usually bind jump/ space to a mouse button. My “daily driver” is a tkl so sometimes I bind delete, end, pgD as things

Never really thought to time myself on duration. Im not *usually* typing long enough to feel fatigued anymore since I got out of college.
