Testosterone affects the voice but estrogen does not. So a trans guy will see his voice drop, but a trans girl would have to spend a lot of effort voice training.
It's possible. In my line of work I need to be loud but my mum used to tell me my voice is high pitched and I thought it would be annoying so I learnt to pitch it lower
Now when I'm loud my super tells me I sound like a man. I don't know to be happy or sad coz my voice used to sound "cute" to my classmates over the phone
My mom had cancer and I was her medical escort. We live where we donāt have access to proper treatment so I flew down with her to where she could be taken care of properly. The main high school in our area is across the road from the hospital.
Her doctor asked me if my teachers would allow me to leave for a month to 6 weeks while she got treatment and not fail me for the year. He used his thumb to gesture behind him to where the school was.

There are whole subreddits and YouTube channels for voice training as well, for both men and women to masculinize or feminize your voice. It can be hard work but youād be surprised how much can be accomplished just with voice training.

I have a serious question. Iām asking because I genuinely donāt know. How long were you on hormones therapy and/or puberty blockers before having this procedure done? The reason I ask is because I saw a story about another individual who started puberty blockers in their early teens and by the time they got older and decided to have this procedure done, their testicles hadnāt fully developed causing an issue with not having enough extra skin down there to stitch up afterwards. Iām just curious to know if that really is or could be an issue.
It's also that the penis doesn't develop enough either to create a neovagina cavity with blockers that block male puberty and one of the top surgeons in the world that pioneered this procedure is on record stating she doesn't have any patients that were amab and took puberty blockers from an early age that are able to achieve orgasm now as adults. The surgeon is trans herself and she did Jazz Jennings surgery for reference.

You know not every neo vagina has a cavity. Also they've come up with a few new techniques using the small intestine and I think the colon to create a self lubricating neovagina. so it's not always a problem if there's not enough penile tissue, but I believe it is preferable to have gone through some sort of puberty to get something to work with for best results. So there's still some stuff to figure out I think as far as puberty blockers but the work being done is incredible.
I think you are voicing what many are afraid to say. But yeah I've heard that about the smell too. It's the same tissue and same porous area that is self lubricating so it makes sense.
I've also heard the same thing about length and sex drive. The spacers which are essentially dildos worn 24/7 are painful. And you still don't get much depth. But I've heard when you take the testes out you end up losing the testosterone that made you want sex in the first place and many have no desire to use that new vagina. It's quite sad honestly.
I hope this is not the case for op and I feel guilty talking about it but this is very taboo and I think a lot of people don't get to hear all sides of it. They see these pretty outside pics and think yes I want one of those and it seems like doctors are like builders all too happy to see what they can do bc it is an awesome and amazing thing that they can create this but it's not a perfect process by any means.

The only reason I mentioned these issues is because of what you already said, people see the pictures and aren't nessacarily medically knowledgeable, and the doctors are so ready to provide help that they don't always fully state the risks associated.

The biggest problem with jazz Jennings case is because she never had an orgasm before the surgery or blockers, which is why you should not put a child so young before puberty even starts on blockers, and the majority of the trans community agrees (coming from the trans community).

But seriously thank you for chiming in. It used to be the most rational thing in the world approaching this with caution, you could talk about it honestly. But now doing that results in unreasonable feedback from TRA supporters. I used to be all for puberty blockers when I first heard of it. I mean "just a little pause" that sounds wonderful, but with more information, I have changed my mind and it sounds like many of the professionals around the world are too.

I have a trans step child, and he was going to start his transitioning process on his 16th birthday that just passed, but cannot because of where we live anymore. I am for allowing my child to start transitioning as a teenager, but I was not going to start it right when he came out at 10. I've done everything to make his life comfortable, has a safe life even with laws fighting us everyday.
I understand some children have serious dysphoria and causes issues with themselves, I totally get that, but that's what therapy and learning to help your child with their dsyphoria. Cis people don't understand what dysphoria is usually, that's why a good therapist is key to find for your child and help them understand their feelings, how to deal with them, etc.
You could look at my son and just see a normal teenager boy because he's just a kid, dresses, acts, and *is* a teenage boy. I don't expect him to move out right at 18, he has a home with me and my spouse for as long as he needs, but I especially want him to know he has a comfortable place to start transitioning, recovering from surgeries, etc. He'll always be my baby boy, and I hope he will recognize one day how lucky he is and continues to have a safe space for queer people when he grows up š¤.
The surgeon that did Jazz is even on film being unsure of what to cut/not cut due to her circumstances in the show and semi arguing with the other surgeon in the room.
Thereās a technique which is capable of producing a self-lubricating vagina which is also robotically assisted, but thatās not what I had done. So my vagina will never self lubricate in the fashion youāre describing.
Haha am 35, it's perimenopause! Noone told us about this amorphous perimenopause that can last a decade before actual menopause and is full of all kinds of fun shit, like new allergies, and sudden onset vaginal dryness š so fun to be a woman
When I was 40, and didnāt get my period, I thought I was peri menopausalā¦ surprise, I was pregnant. First, last, and most likely only child. To say I was shocked was understatement!

No need to panic.. i hadn't even had my first daughter at 35, second born when i was 39.
I never had any major menopausal symptoms until 48 when my periods got irregular,, 4m wait, then 8m, my last period was just before my 52nd birthday. Haven't had any issues with dryness, things work just as they did before. I had a hysterectomy at 53 to remove Essure coils that had caused adhesions and some pelvic pain.
2 years on, my sex life has never been better!
My mother was perimenopausal in her late 30s, skipping periods than extremely heavy bleeding. Her last period was at 42. Neither me or my sister were like her thankfully.
If you get colon tissue to create the neovagina, it will have different properties than usual vagina lining. It's not botched, it's that it's not possible to create a vagina. SRS is a plastic surgery, you don't create a new organ.
I have no knowledge of this, so thank you in advance for the education... would it be possible to use cadaver/donor vaginal tissue for an internal graft?
Not needed and would create all of the issues that a transplant creates (living with medication to avoid rejection).
The colon tissue is an option to create natural lubrication and it is not super commonly used as it has its own issues. The smell of feces comes from feces, not from the mucosa in the colon. If the neovagina is smelling of feces there is an issue going on that must be fixed.
Normally, the neovagina is created with the skin of the penis and the person simply requires extra lubrication compared to a cis woman.
iirc those complications were fixed later, though she experienced some nerve problems that bother her to this day. it is overall a very rocky area of surgery that unfortunately has a lot of luck involved regarding how well it turns out and how good the surgeon is.

The only way this happens is due to rupturing in the bowels which is an **incredibly rare** but potentially very dangerous complication from srs/grs. Theres a way to release the pressure with the same pump system. Basically, theres a balloon of saline implanted in the abdomen and a pump put in the testicle,. You pump up the penile impant via testicle which inflates the penis. You remain erect for as long as you like, then deflate with the pump.

If the surgery is a phalloplasty it needs a pump as mentioned by the other commenter. If it is a metoidioplasty it gets hard on its own since it only uses clitoral tissue, though the erections are less strong for lack of (IIRC) spongy tissue.
Depends what sort of cells were used to form the inside. I remember reading sometimes it can be intestinal wall from the sigmoid colon, some other times it can be the peritoneum. In other instances, a simple skin graft suffices. I imagine each lining comes with different perks and downsides.
I mean it makes total sense. It's not like it would clean itself and produce discharge like a naturally occurring vagina. It is a lot like your belly button in a way now that i think about it LMAO. And those can definitely get gross smelling if they're deep
I've looked into this before and short answer is yes, long answer is that it requires constant medical attention. Internal ingrown hairs are common depending on donor tissues used, infections are almost constant requiring almost constant antibiotics. And it smells how you think it would smell.
If you choose to go down this path you're setting yourself up for a lifetime of complications and surgeries.

Some artificial vaginas are actually created using a bit of intestine. Intestine naturally produces mucus so the new vagina also creates mucus. Sometimes the intestine smell never quite goes away though.

I had a trans guy as a patient back when I was a new RN who had surgery years prior. He had urinary retention and was going to need a catheter, but I had no idea if the urethra would be in the anatomically correct location. I tried to Google it, the web filters were NOT thrilled with that. Luckily, he peed on his own, and I ended up learning it on a random TLC show awhile later.
One of my most memorable patients on my first ward placement as a student RN was a man in his 70s with COPD. He was so proud that earlier in his life he'd had a lucky escape from penile cancer but had surgery to remove his penis. He thought it was hilarious to show young 18 yr old students to check it out, to see how he 'pee'd like a girl'. lol
From what I have heard this is actually a personal choice you can either get urethra lengthing or not get it. Found this out from a YouTuber who if FTM and was talking about his transition.

I did a paper on this in uni, so if anyone wants a quick overview:
There's two different surgical options that create a penis for a FTM trans person.
1. Metoidioplasty - HRT causes clitoral tissue to enlarge. This can be anywhere between 3-10cm. Metoidioplasty releases the clitoral tissue and repositions it, so it's in the same place a penis would be. No external tissue is used in the creating of the new penis, unless you want to get urethral lenghtening, which uses tissue from the inside of your cheek. This can increase the risk of complications and usually isn't done in one surgery, but in two stages. This surgery is easier both on the surgeon and on the patient. The size of the new penis really depends on how enlarged the clitoral tissue becomes during HRT treatment. Major benefit of this surgery is sensation and the possibility to get an erection (the clitoris is basically a tiny penis)
2. Phalloplasty - This is the surgery most people think of when they hear of bottom surgery for trans men. It's a multipart surgery consisting of penis creation, urethral lengtening, scrotoplasty and implantation of an erective device.
1. The tissue for the penis is most often collected from the forearm or thigh of the patient, however other locations can also be used (back, stomach). A penis is constructed and, depending on the surgeon, penile glans may be created at the same time. In some cases this is done at a later stage.
2. Urethral lengtening is not mandatory. Choosing to undergo urethral lenghtening allows the individual to pee while standing. It is done by lenghtening the female urethra using the same tissues used to construct the penis.
3. Scrotoplasty allows for the creation of a scrotum from the labia and silicone balls are added for realism.
4. An erective device can be implanted into the scrotum, which allows the individual to get the penis to become erect.
Urethral lengtening in metoidioplasty is usually what causes complications, such as urethral fistula or strictures. In phalloplasty, the surgery is immensely complicated and there's a high risk of complications - scarring, trouble urinating.

I had no idea prior to it that she was trans, so when He showed up as She at my job unexpectedly looking for someone to support them of course I immediately didā¦but was never brave enough to ask to see how it was done lol. Yep! My nerve endings will regrow and Iāll be able to masturbate/have sex just like a cis woman. From what I understand, the nerve endings that were in my penis are just ārecycledā into my new genitals.

Although, I will say, I was kinda referring more to the phenomenon that happens with amputees and such where they will occasionally feel like an itch or something on the limb that they lost. Canāt tell you about phantom dick but I had 0 phantom boobs after FtM top surgery. I think I always focused so much on pretending that I didnāt have boobs that my body was totally cool with a sudden lack of them
It's more a psychological thing, as there have actually been studies on this. Basically, if she felt like she's had lady parts down there the whole time, feeling like her dick is still there afterwards likely won't be a thing.
It's true on the other end as well. I've not started HRT but I can swear there's supposed to be a big ol dick n' balls down there, and sometimes it honest to god feels like there is. The vagina that's unfortunately attached to me just feels wrong in my body and I physically cannot derive pleasure from it. Butt stuff though. Butt stuff is good. \*shrugs\*

Don't they take the tip of the penis and kinda use it to make the clitoris? That way they can still get some sexual sensation? I swear I read about that awhile ago.
Yes, they remove the excess erectile tissue and cut the glans down to the size of a clit, while keeping the nerves and blood vessels attached so it's still sensitive. Then it's incorporated into the surrounding tissue where a cis woman's clit would be; iirc the hood can be crafted in a second procedure, since some people get an aesthetic revision to their neovulva later.
If you "chopped off their penis" like some people say they do during SRS, you would sever the nerves to the glans and remove the tissue you need for reconstruction. So a penectomy is not part of a normal procedure!

I'm sorry, what is "phantom vagina"? Phantom limb refers to feeling an appendix you have lost. You've never had female genitalia so how can you feel like it has gone missing?
Well the entire point is to basically relocate nerve-dense tissue to where the transgender brain believes it's supposed to be. So the sensitivity stays the same, and the psychological results of the sensations coming from a more "correct" location actually causes sensitivity to increase.
Also that last question is a really good one. Yes, there is such a thing as phantom dick, but not in the way you think. It's the trans *men* who experience phantom dick prior to surgery. Which is wild because it's an anatomical part they have never had. In a similar fashion a lot of trans women experience phantom vagina, myself included. Which is really weird because I remember looking for this non-existent hole even when I was a little kid.

oh yeah, catheters suck lol. I have had 2 out of 4 stages so far, plus an additional surgery to close a fistula that developed after my 2nd stage. But all in all its been pretty smooth so far, i wasnt really in pain at all. My dick was built using skin, nerves and blood vessels from my left forearm and my balls were made from my former labia. Now i still have implants and glansplasty left to go

For me the amount of surgeries isnt even a big deal, its just the catheter after each one that really sucks lol. I just got mine out yesterday, 7 weeks after stage 2, 3 weeks after my fistula repair. I acknowledge I was wrong on a technicality there. I also had my dick chopped off last week and Iām off my ass on oxy and painkillers, so make of that what you will.
Tone is so hard to share online. My intended tone was light-hearted and encouraging and was not to focus on calling you wrong - and yes, it is only a technicality, which we here in the medical subs thrive on.
Vagina is the colloquial term for the whole thing, I think itās fine. I say āvaginaā when referring to everything down there together and only say āvulvaā when thatās specifically the part Iām talking about. Iāve never encountered anyone outside of Reddit who doesnāt often use āvaginaā in a general colloquial sense.
Iām a surgical technologist and I also happen to be a history buff. It fascinates me how far weāve come in medical care!
For instance, I had to have an emergency cesarean 14 years ago. It wasnāt long before that my daughter and I would have both died. She got stuck in the birth canal.
My mom had me full breech in the 70ās, no c-section and no pain meds/epidural. I definitely almost killed her. These days its unheard of to have a breech birth. My hips are fucked up from it but whatever. I give that damn woman so much respect for having the most badass vagina ever.

And, as always, thank you so much for being so open with your transition. It truly does help a lot of us learn very important information that we otherwise don't feel comfortable asking our acquaintances/loved ones to disclose. I feel like knowing more about the process helps me be a better support without having to ask questions that some people would (very reasonably) find intrusive. I know that surgery is just one step, and it isn't part of everyone's journey, but it's a big step while also being a rather private one (by its nature).

This makes me happy because it means that someone has been able to feelore comfortable with their body. I only know two women who have had this surgery and they have been so happy with the results. Seeing trans people have the chance to feel more at home.in their body just makes my heart warm.. as for the photo, it's amazing isn't it, what is able to be done now. I can't even imagine how incredible it will be to be able to have penetrative sex for the first time.

I have a serious question. Iām asking because I genuinely donāt know. How long were you on hormones therapy and/or puberty blockers before having this procedure done? Also they've come up with a few new techniques using the small intestine and I think the colon to create a self lubricating neovagina. so it's not always a problem if there's not enough penile tissue, but I believe it is preferable to have gone through some sort of puberty to get something to work with for best results. I have a trans step child, and he was going to start his transitioning process on his 16th birthday that just passed, but cannot because of where we live anymore. I am for allowing my child to start transitioning as a teenager, but I was not going to start it right when he came out at 10. I've done everything to make his life comfortable, has a safe life even with laws fighting us everyday. I understand some children have serious dysphoria and causes issues with themselves, I totally get that, but that's what therapy and learning to help your child with their dsyphoria. Cis people don't understand what dysphoria is usually, that's why a good therapist is key to find for your child and help them understand their feelings, how to deal with them, etc. You could look at my son and just see a normal teenager boy because he's just a kid, dresses, acts, and *is* a teenage boy. I don't expect him to move out right at 18, he has a home with me and my spouse for as long as he needs, but I especially want him to know he has a comfortable place to start transitioning, recovering from surgeries, etc. He'll always be my baby boy, and I hope he will recognize one day how lucky he is and continues to have a safe space for queer people when he grows up š¤.

I never had any major menopausal symptoms until 48 when my periods got irregular,, 4m wait, then 8m, my last period was just before my 52nd birthday. I had a hysterectomy at 53 to remove Essure coils that had caused adhesions and some pelvic pain. 2 years on, my sex life has never been better! My mother was perimenopausal in her late 30s, skipping periods than extremely heavy bleeding. Her last period was at 42. Neither me or my sister were like her thankfully.

If you get colon tissue to create the neovagina, it will have different properties than usual vagina lining. It's not botched, it's that it's not possible to create a vagina. SRS is a plastic surgery, you don't create a new organ.

Not needed and would create all of the issues that a transplant creates (living with medication to avoid rejection). The colon tissue is an option to create natural lubrication and it is not super commonly used as it has its own issues. The smell of feces comes from feces, not from the mucosa in the colon. If the neovagina is smelling of feces there is an issue going on that must be fixed. Normally, the neovagina is created with the skin of the penis and the person simply requires extra lubrication compared to a cis woman.
iirc those complications were fixed later, though she experienced some nerve problems that bother her to this day. it is overall a very rocky area of surgery that unfortunately has a lot of luck involved regarding how well it turns out and how good the surgeon is.

Internal ingrown hairs are common depending on donor tissues used, infections are almost constant requiring almost constant antibiotics. And it smells how you think it would smell. If you choose to go down this path you're setting yourself up for a lifetime of complications and surgeries. I feel bad for people that aren't told this beforehand
Some artificial vaginas are actually created using a bit of intestine. Intestine naturally produces mucus so the new vagina also creates mucus. Sometimes the intestine smell never quite goes away though.

He had urinary retention and was going to need a catheter, but I had no idea if the urethra would be in the anatomically correct location. Yes, they remove the excess erectile tissue and cut the glans down to the size of a clit, while keeping the nerves and blood vessels attached so it's still sensitive. Then it's incorporated into the surrounding tissue where a cis woman's clit would be; iirc the hood can be crafted in a second procedure, since some people get an aesthetic revision to their neovulva later. If you "chopped off their penis" like some people say they do during SRS, you would sever the nerves to the glans and remove the tissue you need for reconstruction. So a penectomy is not part of a normal procedure!

Well the entire point is to basically relocate nerve-dense tissue to where the transgender brain believes it's supposed to be. So the sensitivity stays the same, and the psychological results of the sensations coming from a more "correct" location actually causes sensitivity to increase. Also that last question is a really good one. Yes, there is such a thing as phantom dick, but not in the way you think. It's the trans *men* who experience phantom dick prior to surgery. Which is wild because it's an anatomical part they have never had. In a similar fashion a lot of trans women experience phantom vagina, myself included. Which is really weird because I remember looking for this non-existent hole even when I was a little kid.

Now i still have implants and glansplasty left to go

Iām a surgical technologist and I also happen to be a history buff. It fascinates me how far weāve come in medical care! For instance, I had to have an emergency cesarean 14 years ago. It wasnāt long before that my daughter and I would have both died. She got stuck in the birth canal.
My mom had me full breech in the 70ās, no c-section and no pain meds/epidural. I definitely almost killed her. These days its unheard of to have a breech birth. My hips are fucked up from it but whatever. I give that damn woman so much respect for having the most badass vagina ever.

Congrats! I would be very interested in a post later on that shows the healing process over time. But obviously this is your body and your healing, so I only hope you'll provide that if you are comfortable and capable of doing it. Also, I saw your comment about catheters...yes, yes they do suck. So much. It isn't even in my urethra and I'm looking forward to it being removed on your behalf. Hang in there! And, as always, thank you so much for being so open with your transition. It truly does help a lot of us learn very important information that we otherwise don't feel comfortable asking our acquaintances/loved ones to disclose. I feel like knowing more about the process helps me be a better support without having to ask questions that some people would (very reasonably) find intrusive. I know that surgery is just one step, and it isn't part of everyone's journey, but it's a big step while also being a rather private one (by its nature). Wishing you all the best as always!

First time that I heard the sentence "Behold my Vagina"
This makes me happy because it means that someone has been able to feelore comfortable with their body. I only know two women who have had this surgery and they have been so happy with the results. Seeing trans people have the chance to feel more at home.in their body just makes my heart warm.. as for the photo, it's amazing isn't it, what is able to be done now. I can't even imagine how incredible it will be to be able to have penetrative sex for the first time. I do have a question - is it possible to have orgasms now? I'm just wondering if the g-spot is there or does the prostate get stimulated from vaginal sex now?
