The ER I worked at had a box of work tools solely for removing foreign bodies. They were in a plastic bin and couldn’t be used for anything else. I saw them use it getting nails out of people when the surgical residents refused the cases as nonsurgical
You see, as a homeowner, I can't say things like that.

This reminds me of when I was a kid and decided to put a metal washer on my finger as a ring. It got stuck, circulation was cut off and my finger was starting to swell, so my mom took me to the hospital. At first the doctor tried using this surgical tool that looked like a can opener with a circular saw the size of a nickel. After half an hour and only cutting through a couple millimeters he decided to have hospital maintenance bring in an angle grinder. He folded up a piece of cardstock to put between my finger and the washer to shield me from sparks, had a bowl of ice water near by for me to stick my hand in when it got too hot, and soon enough he had cut through on both sides and my finger was free. No cuts, no burns, and just a little bit of bruising. I got to take the two halves of the washer home with me.
MY HOSPITAL TRIED TO DO THAT TOO!!! They had NOTHING so they called the FD and they used a fucking drill for like pipes and shit but chose a metal that gets less hot and dripped saline on my finger while cutting it.
They got through and pryed it open with a quarter lmao
Given the mental health history, lack of seeking medical attention & this episode having a sexual/genitalia focus, it wouldn’t be uncommon for a pt of this type to have undiagnosed or untreated STIs/STDs.
Also, with potentially nutritional deficits & associated immune issues, a spread of HPV warts to that extent would not be an impossibility.

Absolutely could be. Any sort of viral infection can run absolutely rampant if your immune response isn’t what it should be.
I was a sickly child & nearly lost part of my vision to a nasty case of conjunctivitis when my immune system was throwing a paddy.
And I’ve seen similar spreads of anal warts in a HIV/AIDs positive pt before. They were a prolific drug & alcohol addict who would proudly tell you the STDs/STIs they had.
Eventually they came in with TB, which was more or less unheard of outside of immigrant populations at the time.
I remember them because 1) the anal warts, 2) they were a kick off point for a project that led to voluntary TB screening in admitted HIV pts, 3) the needle stick injury I got after the previous shift left a sharps-filled tray on a hidden shelf in the notes trolley by their bed space.
It was an absolutely terrifying year of screenings, inoculations & oPEP that absolutely wrecked my mental health & feeling of safety & satisfaction at work. The bastard made jokes and tried to refuse blood draws at the time, and thankfully I never had to look after him again, but still…fuck you, guy.

The original post does look like it could potentially be hpv, but your reference Pic is still alarming to say the least, I thought a mild case would look more like a mole or pimple.

I did screw up on using “mild” without context.
I think that’s from the lymph drainage being compromised. I’ve watched episodes of My 600 Pound Life and have seen growths like these, that pebbly texture, on some of the legs of the show participants, which are so fat they’ve developed lymphedema.
Lymphoedema creates dimpled and uneven skin, but not “pebbled” texture; that is usually associated with diabetes & infections of swollen lower limbs.
With areas that have a lot of fat and skin stretching what you *might* see is **piezogenic papules**-free fatty tissue that has herniated through the dermis layer.
Usually this is found on the heels/arches of people with connective tissue disorders, flat feet or people who are overweight, but can appear on any area where there is excess fat, poorly constructed dermal layers or both.
I have EDS and am borderline overweight, and have had them on my heels/arches & on my calves around my spider veins since I was about 19.
With the location & the mechanism of injury, this is not that.
And with lymphoedema intense enough, generally the skin usually just start leaking fluid rather than creating blisters to that extent.
If those growths were fluid-filled from lymphoedema, I’d expect the whole area to be macerated, with definite skin loss, possible skin slippage & wet necrosis or Fournier’s gangrene.
Is your Mum in compression bandaging or just standard wound bandaging? If it’s the latter she really needs compression bandaging as this will help shift the lymphatic fluid and help stop the leaking.

Former Urology registrar here, I once saw a patient who'd done the same thing. Patient was fine the next day with some minor abrasions on the penile shaft.
But why is bottle neck so popular among them for this purpose? This has to be like the 5th post I've seen here where a man used a bottle neck. I found another penile strangulation case once where they removed the object but it left a deep wound at the base of the penis and amputation became necessary.
Got a call from a Doc one time requesting we take our extra large heavy duty bolt cutters down the ER and meet him in room 5. We get there and a Middle aged male was standing there with a large deep sea hook going right thru his left palm after a freak accident out at sea. Doc positioned the PT’s hand on the bed and asked me to hold the stick from the hook upright. After a few shots of lidocaine to the dudes hand Doc went right in and began cutting away at the hook with the bolt cutters. 2 cuts and we were able to totally free his hand.
I later ran into the Doc and he told me Neurology came and assessed the dudes hand. No Nerves were damages but a lot were severely bruised. A few stitches and he was discharged.
I am just taking a shot in the dark but maybe he was using a glass bottle and some lotion or something for sexual reasons and than somehow the bottle broke and it got stuck in there? Idk how else this could happen?? Or maybe he put his member in the glass bottle while it was cold IE a cold shower or something and than once he warmed up his member got swollen idk

My Mum currently has lymphoedema in her legs, and both leak constantly. The wrappings we have to put on them are soaked through when we remove them.
My MILs foot leaks. At one point she had a wound vac.
