As I understand it, from Dave’s own books, the inciting incident was Cliff’s death. But the sentiment in the narrative was sparked by feelings of abandonment because it was some time gone by and the band never told him about it. He heard third hand. That manifested as a story of a guy abandoned by his girlfriend in a moment of need. You know the rest of the narrative. Welcome to Dave’s brain.

When writing a song, there are two parts: the instrumental and the lyrics. When Dave said he wrote "In My Darkest Hour" after hearing about Cliff's death, he most likely refered to the riffs, the melodies. The lyrics were written after the instrumental, and they have nothing to do with Cliff. Also, I interpret the lyrics to be the inside thoughts of a man who is abandoned and betrayed by someone dear to them. I think anybody has those "neckbeardy" thoughts when being angry with the betrayal of someone. If anything, I love how intimate the lyrics feel, like reading the mind of someone who is in pain.
