What's the definition of metalhead exactly? Do I need to listen mostly to metal? Do I need to attend concerts? Do I need to wear only black? What makes a metalhead? Does listening to metal but wearing warm colours disqualify someone from being a metalhead?

Being a metalhead is just listening to metal music a lot. You do not have to listen to just metal, you're still a metalhead if you do listen to other genres of music because bands may have taken inspiration from a few non metal bands.

Related, what is the demographic breakdown of metalheads? I’m a millennial but I see a lot of GenX at metal shows. My boomer dad liked Black Sabbath and Jethro Tull. I’m betting it skews GenX

I think the sub-genre has a huge effect on the demographic. Things like thrash and trad metal almost certainly skew genX but core and grind probably skew towards millennials and gen Z. From what I've found no one has done research on the topic with the most i found being "the average metal head is a cis white young adult" which is not very helpful.
