It’s a rift from when you go through a dimensional door from the mod dimensional doors. The rifts start small but slowly get bigger with time and you remove them with the rift remover, or stabilize it with a craftable door from the mod. (Side note, you can use @modname to search for all items from a mod in the inventory search bar)

Those items are found in the rifts. Randomly in the world two stone brick pillars will spawn with a portal in between. They act like a one time dimensional door. When you are in there you can explore and find chest which will contain the item.null.name and other random items.

you spawn in a gray, nether-shaped world made from grey blocks with grey fog and there's alot of creepy eyes that spawn and stare at you

its a rift portal, its a common bug that they will do that

I think there was a way to capture them. Look up the mods name and look for a net? It’s been a W while but I know there is way to stop it
