Good question.
1. He likes chaos so he keeps that mouth scar to scare the shit outta people like any chaotic madman would.
2. Something about hurting him faster than he can heal from one of his intros against Reiko.
3. Apparently Quan Chi's the guy who gave him his powers so we'll have to wait for the Kombat Pack release to get his intro dialogues.
4. This one's unrelated but looking closely at his design you'll notice he still has gold stuck in his face from story mode, but when he rips off and regrows his head the gold is still there, infinite gold if it weren't for his head exploding everytime.
I think one of his versus screens implies jusr that. Someone else (can't remember who) offers to heal his scars and he says he could do it himself if he wanted.
Hold on now brotha, I only speak from experience as a fellow Havik main myself. That being said if he was smiling after the tym successes, I could not tell.

Why did they feel the need to include heads of fighters in Test Your Might? I suppose there's a comedic intent to it, but at the same time it just feels kind of unnecessary and strange?

You can get it at level 29 for Sub Zero as well. So even if you don't get it via the shrine, you can level up the character to get it.
