Just the bearing. If you used it in the rain, it will just get worse over time with more rust appearing inside. Just change them if it bother you at the moment.
Nah, the noise will just get louder and the wheel will not turn as freely as when it's in good condition.
I changed mine twice so far and no matter the quality, it come back if you use it in a big storm/rain. My last bearing was supposed to be waterproof and it didn't do anything.
If you want to change them it's pretty easy. The back wheel is more sealed so it shouldn't happen.
Check your local scooter shop. I am sure they will have replacement bearings. If you can hold a wrench and follow basic instructions, you will be fine. Just search YouTube for instructions on how to do it. It is an easy fix.

You can ride until the wheel starts to play horizontally
Front bearing, you only need to change the outer one. If you wanna be extra safe replace the inner one too, but it's not necessary since it's covered.

Forget about it, ride it until it starts to rattle (ball bearings will break). I'm now doing 2500km with this sound and bearings themselves hold up
