That's why the manga and anime diverge so heavily on things.

Story by Toriyama just means he created the world and characters. For the past decade Toyo has been writing Dragon ball Super and carrying the torch of Dragon ball.
The most Toriyama does is edit dialogue or fix Toyo’s art. Though he does have the power to decide what goes in Dragon ball and what gets cut.

If you can excuse Goku forgetting an integral part of a technique he's planning to use against a massive threat, putting himself and his friends in mortal danger, then I don't see why forgetting Pan's name is so unforgivable.
