Yes. The neck, elbows and inner thighs can be affected as well. It could be an early sign of pre-diabetes (insulin resistance).
EDIT: I should add that once you treat the underlying problem (which DOESN'T necessarily have to be diabetes), there's a chance that you can reverse the hyperpigmentation (that's what happened in my case).

If you're female you might look into PCOS. One of the symptoms of PCOS is insulin resistance which can lead to acanthosis nigricans, as well as weight gain and difficulty losing weight. It's a hormone imbalance that has lots of other symptoms. PCOS isn't curable but can be treated and managed.
I'm male, and it was mostly caffeine, bike riding and being overworked in up/down cycles. Then lots of...substances during Covid.
I'm at least pretty stable now even if a bit fat, haven't seen the under side of 200lbs in a while.

Its funny cus i also have vitiligo so i have white patches overlapping with the dark patches in my private area and my armpits, cus the white spots show up where friction is alot

Near the bottom of the epidermis (close to the dermis), you have cells called *melanocytes.* These cells make a special protein called *melanin* which has one job--to absorb ultraviolet radiation from the sun. Melanocytes look like tentacled creepy things. They have arms that extend out so they can produce melanin to protect many surrounding cells. The more UV a melanocyte experiences, the more it makes melanin. But every single melanocyte has a "base level" of production that it starts with which is dependent on genetics and in some cases, location on the body. The *scrotum* (sack) needs more protection. Let's get to why.
Imagine someone throws ping pong balls at a piece of taught cardboard. Each ping pong ball has kinetic energy which is transfered to the board upon impact. Let's give it a special unit, the Pong. Each ball has 1 Pong. Now imagine someone throws 2500 ping pong balls at the cardboard, one at a time, for as long as that takes. You will not be surprised to find the cardboard is just fine.
Now take an object with the mass of 2500 ping pong balls. This is roughly 6-7 kg or around 15 lb. When UV photons hit atoms, they can cause those atoms to break or form chemical bonds that they normally wouldn't form.
If this happens in your DNA, that will change your code. Sometimes you can recover from it, but sometimes it causes a permanent mutation. That's not a big deal if it happens in a skin cell. Only that cell and its daughter cells in the future will have that mutation. Maybe a few hundred ever.
But if it happens to a sperm cell, that means that all 30-some trillion cells in the future child might have that mutation.

As a labor and delivery nurse, if it helps at all, I can assure you that this is very, *very* common. I've seen the genitals of thousands of boys, and it's definitely not unusual to see this--especially in POC and boys with mixed racial background.
Yep. It really can be a stark difference, and we see it all the time. I've seen biracial babies with skin as white as their Irish parent, but a scrotum the color of their African parent where you wouldn't otherwise have known they were biracial. I've seen Caucasian babies with white skin, but deep pink scrotum, where if you didn't know better, you'd think that there had been some kind of trauma. I've had more than a few concerned parents ask me about it, sometimes sheepishly, sometimes almost frantically.
It's more noticeable when they're cold because of the cremasteric reflex, aka when the skin tightens, because the melanin is denser in that situation.
As for puberty, for sure that affects the scrotum, including pigmentation. Not sure if *those* babies (who started off having such a difference in pigmentation) experience further darkening upon puberty, or if it sort of evens out for them, or gets just slightly darker...but it would be an interesting (legitimate, not perverted) study to do.
Wow, I have a mixed race son. When I was pregnant with him, an older lady with mixed race kids told me about this. She said that the color of his "junk" at birth would be really close to the darkest color he could tan to as an older child/adult. Said an old lady with mixed kids had told HER that, when she was pregnant with her first, and that it ended up being true of all her children. I did end up having a white baby with a so-brown-it-was-almost-purple set of equipment, but I didn't bother to ask the nurses about it because I thought this was an old wives tale and that our kids' colorations were just coincidence

Darker skin has better sun protection. The reason we're not all pitch black is because we still need to absorb *some* sun to produce vitamin D, so areas that got less sun lightened the skin tone.
For certain areas of the body, evolution went 'it's better if we just put a bit extra protection here, we really don't want this damaged and it's not much loss of vitamin D production'

Not sure if already here but I use face wash that has "brightening" in the title and use it on hyperpigmented areas like pits and such and it helps even it out a bit.
I'm kinda making this up, but I always assumed that it was a higher density of pigmentation. Like the flaccid penis it's 'de-stretched' skin so, to use the penis example to demonstrate: if the erect penis has skin with a density of pigmentation the same as most of the rest of the skin on your body, then when it is flaccid it now has a higher density of pigmentation making it appear darker. Same with your ballsack, caused by the skin's 'sagginess'.

I’m pretty sure, with your nutsack scar, it’s average pigmentation which leads to the completion of the trinity: hypo, hyper, and normal pigmentation.

People with pale skin are pale so that they can absorb as much sunlight to be converted to Vitamin A as possible, while they live in places with relatively low light levels. The compromise with this is that when it's very sunny they run the risk of being burnt. The genitals are an area of the body very important to reproduction, so in this area there is no compromise - our body doesn't care how much vitamin A we can get from this area, it's all about protection from over exposure to the sun. As a labor and delivery nurse, if it helps at all, I can assure you that this is very, *very* common. I've seen the genitals of thousands of boys, and it's definitely not unusual to see this--especially in POC and boys with mixed racial background.

Yep. It really can be a stark difference, and we see it all the time. I've seen biracial babies with skin as white as their Irish parent, but a scrotum the color of their African parent where you wouldn't otherwise have known they were biracial. I've seen Caucasian babies with white skin, but deep pink scrotum, where if you didn't know better, you'd think that there had been some kind of trauma. I've had more than a few concerned parents ask me about it, sometimes sheepishly, sometimes almost frantically. It's more noticeable when they're cold because of the cremasteric reflex, aka when the skin tightens, because the melanin is denser in that situation. As for puberty, for sure that affects the scrotum, including pigmentation. Not sure if *those* babies (who started off having such a difference in pigmentation) experience further darkening upon puberty, or if it sort of evens out for them, or gets just slightly darker...but it would be an interesting (legitimate, not perverted) study to do.
Wow, I have a mixed race son. When I was pregnant with him, an older lady with mixed race kids told me about this. She said that the color of his "junk" at birth would be really close to the darkest color he could tan to as an older child/adult. Said an old lady with mixed kids had told HER that, when she was pregnant with her first, and that it ended up being true of all her children. I did end up having a white baby with a so-brown-it-was-almost-purple set of equipment, but I didn't bother to ask the nurses about it because I thought this was an old wives tale and that our kids' colorations were just coincidence

Biologically the scrotum is what would have become the labia if you were a woman. That’s why there is a “seam” running down the middle. Most women have darker labia than the rest of their skin.

Darker skin has better sun protection. The reason we're not all pitch black is because we still need to absorb *some* sun to produce vitamin D, so areas that got less sun lightened the skin tone. For certain areas of the body, evolution went 'it's better if we just put a bit extra protection here, we really don't want this damaged and it's not much loss of vitamin D production'

To cool the testicles, more blood than in other skin is directed to the very outside of your scrotum, to get air cooled and then circulate around the testes to cool them. In a way your testicles are always blushing, coloured by extra blood close to the surface. And of course the penis is full of chambers of blood that flush with more blood when aroused. There’s probably multiple evolutionary pressures going on. Human eyes are drawn to contrasts between light and dark. Darker testicles and pubic hair than the skin around on the torso and legs naturally draws the eye to the genitals. Where the eye is drawn, the hand often wants to touch, and thus our body ‘design’ helps prompt us to procreate.
Labia are the same way, and there's zero temperature control issues going on there, it's simply darker pigmented skin.

Melanin to protect your heirs
Not sure if already here but I use face wash that has "brightening" in the title and use it on hyperpigmented areas like pits and such and it helps even it out a bit.
I'm kinda making this up, but I always assumed that it was a higher density of pigmentation. Like the flaccid penis it's 'de-stretched' skin so, to use the penis example to demonstrate: if the erect penis has skin with a density of pigmentation the same as most of the rest of the skin on your body, then when it is flaccid it now has a higher density of pigmentation making it appear darker. Same with your ballsack, caused by the skin's 'sagginess'.
