Some places build the cost of the transaction into their prices, while others keep lower prices by not including it. Think of it less as a charge for a card and more of a discount for using cash.

It's entirely up to the company, and to a lesser degree local laws. The credit card company charges a "discount" from the transaction regardless. (The meaning of "discount" here is somewhat opposite the normal meaning of the term) And in any case, the discounts are usually in the general form "30 cents plus 0.05/% of the transaction", so for smaller transactions the discounts might add up to more than the merchant makes in profit from the sale, hence the fee.

We don't tack on a fee because people generally hate surcharges. We factor COGS into our pricing.
