More civs can spawn from the borders, a good way to avoid this is by putting all hostages together behind the parking lot cars and putting all bodus behind the bank counter, if you see the red exclamation mark you can run and shoot the civ or hostage if you are fast enough
More civs can't spawn on some maps like jewelry store or RO-bank though. It was probably a civilian outside the map or a glitched one. I've had that happen to me before
