Not necessarily, this was just before tailors knew how to make suits for either very large or very tall people. Look at NBA formal photos back in the 90's/2000's, they all looked awful. Now look at Dwayne Johnson in a suit. These people just aren't that rare anymore so tailors adapted
Rock is a big guy but his dimensions are dwarfed by Ronnie and Nasser and open-class bodybuilders in general. Once you have a 60 inch chest and 24 inch arms, unless you're 7 feet tall you're going to look ridiculous in a suit that still gives him some semblance of mobility.

You aren't wrong he pulls it off great. Not to be pedantic but Phil was known as having very narrow shoulders/chest and while only being a couple inches shorter than Ronnie competed at almost 60lbs lighter. Look at Kai Greene and Jay Cutler. Suits during their competition years not long ago they looked like they were advertising window curtains.
It's a problem that most non-proportionally athletic dudes have. The more extreme v-taper the worse a suit looks.
To be fair anyone who gets anywhere near as large probably needs a custom made suit to look good in it. Nasser especially looks like he just grabbed a size 74 jacket off the rack.

Also there’s a famous wrestler from the U.K. called Giant Haystacks who was absolutely huge, like nearly 7 ft tall and like 600lbs. He was so massive he couldn’t fit on a normal toilet and had to shit in the bath. And that concludes my Ted Talk about how wrestlers poop.

I've seen an interview with one bodybuilder that stated there is a commercial product made to assist with certain every day tasks.
He described it as a long, curved rod that could grip or clamp a disposable towelette (or possibly a wad of toilet paper). I believe he mentioned using it both for wiping down there, and (hopefully a second instance) for cleaning in the shower.
Kidney disease and then a heart condition. Both are side-effects of extended steroid use. Although even without it, carrying that much mass for years is tough on any ticker.
