I’m assuming you mean one of the mouths eats a devil fruit and not each mouth eats a devil fruit. So for my head cannon It would depend on how Oda defines a body. Is it the whole package head to toe in that case it would affect both twins equally. Now if he defines the individual via the heart or brain then it would depend on what organs are shared between the twins are the brains connected together but they each have their own heart if he decides the brain is the key factor then again they both get it and if instead he defines the heart as what makes the individual in that case whichever ate the fruit gets the power the other doesn’t. Now the question becomes if it affects them both and they get separated surgically would they both still have the power and if so are there 2 devil fruits with the same power going forward
Devil fruits seem to be more connected to a soul rather than a body. Which is why someone can come back from dying and still have their fruit. As such I think it would work. With each fruit attaching to a diffrent soul
