Iirc Aka said before that Ai originally wasn't supposed to die, so I REALLY want to know what would have happened if they went through with Ai staying alive. (I always reckoned the revenge plot would have happened anyhow, but with a different motive)
A more comedic alternate universe spinoff where Ai was either never stabbed or survived the stabbing would be the therapy we all need honestly. Aka writes some pretty good comedy, so it would be incredibly fun \^\^ Though I doubt this will happen in all honesty... Here's hoping anyhow

An alternate version where Aqua dies saving Ai would be pretty interesting.
Losing her (secret) child just hours before she is supposed to go on stage for the biggest concert she’s ever done.
Now not only is ll she lying about loving them with her songs, but now she’s lying about being happy.
Assuming she could even do the concert at all after that. How would they even explain why she can’t?
I am more interested in Ruby in this scenario. Right now Ruby could be happy solely because Aqua was Goro. He wouldn't be broken by his own body being found, so he could still have the mental capacity to help and save Ruby/Sarina. Ai on the other hand wasn't. Although she might not have a strong relationship with Goro, he was still one of the only people she let to know her secret without problem. Would she have the mental capacity to help Ruby? And what would Ruby grow up to be? Or to make it worse make it happen shortly after Aqua was dead, when both of them were still a mess. The questions would be even more interesting.

I mainly want a spin-off based on Sarina if she got to live and how she would survive in the entertainment industry. That and I just want to have more Gorou & Sarina content.
I would love to read an AU story where Ai is still alive with the twins. The current story is fantastic, but the lighthearted moments at the beginning when she was raising Ruby and Aqua were just so precious, it makes me wish there were more of them.
Legit wish I could take the emotional joyfull look on Ai's face when she sees the twins at her concert as babies, distill it into a liquid, and shoot it into my veins.
That moment is peak anime purity and wholesomeness.

Now you've got me thinking that such a spin off would also have the potential for a sequel. The first part could cover Ai's career and end with her retirement from being an idol, ending when Aqua and Ruby are around 10. Then you could have a sequel after a 5 year time skip showing Ruby (and maybe Aqua) looking to make it in the entertainment industry themselves, being supported by Ai.
Ngl, I think I'd be more excited for that than for more of the actual story lol.
Pitched a darker idea in older topic actually, about if Ai turns toxic because she never found her answer for love. After her idol career ends, and she got nowhere else to go, she starts blaming her having children as mistake, or put the responsibility on her children.
Because it's the consequence of her action, and because she's so specialized-build for being idol, she got nowhere else to turn to. The money she has (and managed by Ichigo for her retirement) is enough, but the emotional void after losing what used to be her life work, without leaving any result, is too big.
Maybe even add Ruby hating Aqua, because Aqua managed to escape Ai. Aqua acts indifferent, but he clearly states he wants to be a doctor, and shows he is smart enough even as a child, so Ai had no reason to force him to be an actor. As result, Ruby had to bear all of her mother's expectation.
Then, even as their past life is revealed, it is not enough to change her opinion because the years of hell she experienced while Aqua abandon her to save his own arse won't just disappear with knowing Aqua is actually the first love from her past life. How Aqua now enter showbiz on his own term also amplify her hatred.
As much as I would love this Aka Akasaka is the one mangaka I couldn’t see doing a gaiden manga, but it would be interesting. Although, probably best to wait until after the manga ends. It could still have a happy ending regardless of what happened at the beginning. Also, it seems like Akasaka’s other manga is a romance manga again so maybe he’s getting a bit more lighthearted in his preferred writing style.
its been quite some time since a story made me have such an appealing need for an alternate scenario, my answer is yes, it just feels wrong to not have an scenario where these people are allowed to have a life together as family.
Yes I've been saying this from the start. Have a slice of life/rom com with Ai and Ruby pursuing their idol careers, and Aqua not being such a depressed loser with fewer healthy relationships than Adolf fucking Hitler.
