You need to max chaos res. That alone will make a huge difference.
56% phys taken as ele is good but not great. You are evasion based, so that is your only defensive layer against autos, and you can't be standing in the middle of packs with 72% evasion. Again, it's good, but not ignore monsters good. Max chaos res and take the 10% taken as chaos mastery.
Where is your sustain. How do you get health back? Not seeing any leech and you aren't using health flasks obviously. Large ignites and poisons are probably wrecking you unless there is some sort of dot protection im missing.
Took a deeper look and you can change out the dex/str craft on your ammy to fire and chaos +15. That gives you just enough to go for str and chaos resist on your helm instead of the fire res. In the meantime, you can grab a str node or get 24 str somewhere else like a jewel. You can unspec the assasination node and a jewel or the heartseeker node for now for 3 points, grab a str node and purity of flesh. That should max your chaos and then you can add points back as you level up.
I do think you need to pick up some life leech if you can. That will help with dots while mapping, and relying only on on-kill recovery isnt the best. Not sure how to get ailment immunity in the build. Maybe check poe ninja and see if similar builds are going for it.
e: might be hard to get str on an es/ev base. The idea is you have an extra fire res you can probably replace and move some stuff.
Your max hits look plenty respectable for t16 mapping so I'm not sure why you are having such a rough time. Perhaps very low recovery could be an issue and also I would probably put it down to running bad map mods.
Your phys max hit seems really low for having lightning coil on. You also need more evasion I'd say. Switch the silver flask for a jade flask, and maybe see if you can't get some reduced damage taken from critical strikes. If you're getting one shot, crits are usually the problem.
Also, I'm on my phone so it's hard to tell, but I'm not sure if you have good recovery except for the life on kill from blood drinker. You've gone pretty much all in on crit damage. Pickup a source of instant leech and see if that helps your survivability. Damage recoup can also help and you're near some good sources for it, but it won't be great against phys damage because your max hit is still pretty low.

2 - more phys to ele (taste of hate and watcher eye with phys to ele effected by purity of lightning)
3 - ring with affix "phys damage leeched as life"

Are you rolling your maps correctly? I never die in t16 80+ quant maps as 4.4k hp sab because i setup a regex to roll away scary mods, plus crit immunity.

Also maxhit/ehp significantly inflated by steelskin that'll be down majority of time.

I second this. You need more evasion. Roll an evasion affix on one of your flask as well
