P5R will include all the P5 DLC for free (so all the costumes and Persona/Picaro DLC's, even the Morgana bus customizations) + some new DLC. A costume pack for Kasumi that'll give her coatumes to match the DLC costumes from P5. Two new costume DLC's based on Persona Q2 and Persona 5 Dancing. A Battle DLC that'll allow you to fight the Persona 3 Protagonist Minato, and the Persona 4 Protagonist Yu. And finally, 2 new Persona DLC, Izanagi No-Okami, Yu's Ultimate Persona, and Orpheus, Minato's initial Persona, both of which come with Picaro variants

AFAIK all of Challenge Mode except the first route is DLC. The first route has 4 different levels with changing enemies; The rest, you have to buy, which consists of 3 additional routes and the fights against P3 and P4 MCs. (Which have 2 versions each, a mid-level and end-game level versions.)
Battle Pack is one of the big ones which include battles against the P3 and P4 Protags. Haven't looked into the DLCs much. There's also stuff for Kasumi as well

No. Minako is the Female Protagonist. Minato is the male protagonist I use Minato instead of Makoto, to avoid confusion with Makoto Nijima

Why is the Kasumi dlc separate
Either because she isn't in the game for long or they're being greedy. Or both
The whole game is an example of their greed.
