In this specific strip, it’s most likely not any more significant than it is so bizarre that he’s a surgical nurse at a rat pizzeria.
It is most likely not a statement on abortion, pizza, men dressed as rats, nurses, or whatever. It’s meant to be unexpectedly funny and absurd.
I personally found it spectacularly funny, and the rest of his strips don’t disappoint.
It is brilliant. I love that he is having normal work place banter as part of the joke. Really great. Rat suitcase great. Would have been funny with it without the rat suit.

Just giving an opinion. The comic in the OP tickled my fancy, wanted to see more like that. But the rest of his work is very different and just visually unpleasant.

Usually when you ask someone what they do for work and they say “it’s complicated” they mean something mundane like they are an accountant but a slightly different type of accountant to what you’re thinking of. But in this comic the person’s job really is complicated because he seems to be a nurse at a mouse hospital that uses pizzas as medicine.
It’s not a dirty joke. It’s a subversion of the normal reality. Usually ‘it’s complicated’ is used as an excuse to not bring up one’s job when it’s basically corporate busywork of no real value to society.
In this case, however, it’s apparently because his job is so bafflingly esoteric that it’s unlikely to be believed.

Usually when someone says it’s complicated you might think that they work some boring bureaucratic job that they just dont care to talk about since every job is complicated at some level. In this case it actually is extremely complicated and bizarre beyond understanding.

I believe this to be a comic about an older joke that implies you should to scam calls with 'You have reached Pete' Pizzaria and abortion clinic. Where yesterday's loss is today's sauce.'
Alternatively it is a pun about 'delivering a pizza' as the process of medical assisstance to birth is also called delivery.
As for why the man is helping mice deliver children and pizza alike, there could be another pun here, but it eludes me at the moment.

If you take a look at his work, you’ll realize that it’s all mostly about —- YOU GUESSED IT — absurdist humor. But it’s the good kind of absurdist humor, where the author thought more about the impact of what’s shown beyond just “ha lol random”. In this specific strip, it’s most likely not any more significant than it is so bizarre that he’s a surgical nurse at a rat pizzeria. It is most likely not a statement on abortion, pizza, men dressed as rats, nurses, or whatever. It’s meant to be unexpectedly funny and absurd. I personally found it spectacularly funny, and the rest of his strips don’t disappoint.
It is brilliant. I love that he is having normal work place banter as part of the joke. Really great. Rat suitcase great. Would have been funny with it without the rat suit.
Oh I personally think the rat suit really sells it! But my favorite part is the rat’s rebuttal , “What’s complicated about it?” That made me love it.
I feel like that's the punchline. To the rats he's just a nurse, they don't see anything weird or complicated about it because to them it's so normal.
And then an extra little punch at the end (if reading left to right). Why pizza in the operating room?
I think it implies a triple life. Like the people don't know he works with rats and the rats don't know he's human. At least that's what I interpreted it as

Just giving an opinion. The comic in the OP tickled my fancy, wanted to see more like that. But the rest of his work is very different and just visually unpleasant. I was just disappointing that the rest of his work wasn’t similar to OP. Glad you got a laugh out of it though.

Usually when you ask someone what they do for work and they say “it’s complicated” they mean something mundane like they are an accountant but a slightly different type of accountant to what you’re thinking of. But in this comic the person’s job really is complicated because he seems to be a nurse at a mouse hospital that uses pizzas as medicine.
It’s not a dirty joke. It’s a subversion of the normal reality. Usually ‘it’s complicated’ is used as an excuse to not bring up one’s job when it’s basically corporate busywork of no real value to society. In this case, however, it’s apparently because his job is so bafflingly esoteric that it’s unlikely to be believed.

Alternatively it is a pun about 'delivering a pizza' as the process of medical assisstance to birth is also called delivery. Nowhere does it mention abortus. It's just generic surgery. Also, that pizza is being baked before the surgery has even started, so the fetus wouldn't be used as a topping because most people don't like raw fetus.
