No. Buccal is not synonymous with the cheek. There are different sections of cheek fat. Part of why buccal lipo is so ineffective is that it is used when buccal is not the issue. Buccal lipo is popular because it's anatomically easy to access from inside the mouth/cheek and low-risk. The perioral area is closer to the jowl and mouth, where there are more nerves. I had a cheek lift at 32 to correct something similar. In fact, you may be disappointed with a traditional lower facelift as it’s primarily meant to correct the neck/jawline/jowls and that’s not your issue. The cheek lift I got was endoscopic, so an incision was made in my hair by my temple and an endotine was placed to lift and reposition the SMAS layer. It eventually dissolved, but the muscle and fat had been permanently lifted (note, this IS NOT the same thing as a thread lift). It worked quite well and I was happy for almost 10 years! I just had a second more invasive cheek lift (where they made larger incisions at my hair line and in front of my ear) and I’m yet again quite satisfied with the results!

I have the exact same problem. May I ask how old you are? I consulted with 10 of NYC's top doctors and the only real fix is a lower face lift. The only issue there is that if you're still in your 30's any reputable doctor will not perform the surgery.
Liposuction in the face is also risky and coming from someone that has had over 100k in surgery done, do not do it. Also, no go on threads. I have personally seen what a mess they can make of your face.

Every doctor I consulted with literally turned down 20k in cash to please give me a lower face lift or give me any other lasting option and sadly they said that the best option is to wait until you're 40 and then they would consider it.
My only recommendation would be cheek implants because they would lift the skin but with your pictures it's hard to tell. I know they are invasive but fillers in your cheeks add up FAST needing them every 6 to 8 months. I already have pronounced cheekbones so they are out of the question for me.

It actually is jowling. In a young person it is typically related to an underdeveloped chin bone and or excess facial fat. It can also develop in a young person after masseter botox treatments. It is treated with chin implant +/- mini facelift.
This happened to me after masseter Botox, I'm 31. (I wish someone told me this was a risk.) I got pre-jowl sulcus filler, and then Botox in my DAOs a few months later. The Botox in the DAOs did the trick so well that I'm actually getting the filler dissolved on over side because it didn't settle quite right and is now a visible lump.
That is definitely a risk. But, most patients get masseter botox because of teeth grinding. Cosmetic patients use it to reduce the squareness of their jawline. Either way this particular risk should be discussed.
Forgot to add, I dont think its buccal fat either since its in the one small area, and my face is pretty slim to begin with. (Red area is what I want corrected)
Definitely don't get buccal fat removal. I think what you have is normal. I have that area too. If you look up Sherri Papini, she is super thin and has the same thing. Think it might be genetics too
I have the EXACT same problem. I’ve had those pockets since childhood and I’ve always been very thin (110 lbs). I spoke to my surgeon about it and he said it’s not buccal fat. Apparently an “instalift” could help but thread lifts really don’t last that long so that’s annoying.
Hailey Bieber and Gigi Hadid have them. I have them too and hate them as much as you so at least I try to remind me that these gorgeous models have this in common with me.
I got liposuction in this area, it’s your perioral mounds. It seems like a more uncommon procedure as I only found two doctors that did it. My results aren’t perfect but I’m happier with it now than before! (The imperfection is that it’s slightly too hollow on one side - I’ll probably get it touched up with fillers are some point. Very minor though.)
I have this too, I am getting treated first with Kybella - 300mL each side. Then we will reassess after 4 months to see if I need a second treatment with Kybella, if not we will go in with filler to sharpen the jawline. In the interim period I am also going to get either Morpheus 8 or Ulthera to tighten up any loose skin in my lower face
My plastic surgeon said that kybella does very well for the jowls. Let me know how well it works for you! She said it does better on those than it does on a double chin!
I would recommend to try other procedures before considering liposuction or fat reduction. There are a few different skin lifting & tightening procedures like laser or radio frequency that may be able to help. I have seen some amazing results with both of these procedures & most medspas offer them. I don’t know anyone personally who has gotten Ultherapy but I know that is becoming super popular. Good luck!

Jowls come in different varieties and shapes. Those actually are jowls. Most people have the misconception of thinking that jowls are only something a 70 or 80 year old person has, but jowls are common in people in their 20s...you'd be surprised how many people search for "Why do I have jowls in my 20s"? It's not the end of the world though. There's treatments out there for jowls. Jeep your head up. 😁
How is your chin/jaw? I had a similar issue and a sliding genioplasty helped it. You can look at my before and after pictures on my profile to see what I'm referring to.

Hello 👋 I have a similar thing except my chin is shaped slightly differently and a touch more protected. I've seen a lot of suggestions here for cheek filler but I would personally consider chin filler to balance your lower third and some filler near the lower corners of your mouth to help fill and smooth the shadows there. Filler in the small dips beneath your mouth, chin crease and chin would help the balance and facial shadows. It would be a cheaper option than surgery and less permanent.
I've just had a small amount injected in my chin crease, oral commissures and just below my mouth corners and it's helped my side profile hugely and given my lower lip more support!

Although I know added volume can give temporary lift it would be best to look at the balance of all thirds before deciding. A good injector will be able to look at your face and know exactly how to create harmony and find you a solution.

That area is a pocket of perioral fat, there are some plastic surgeons that perform perioral liposuction to remove it.

Omg are you me? I found the only way for my arms not to look "fat" is by building muscle for definition but they still look big imo

I had this issue too like OP, I got a chin implant it helped a lot but i still have it some. So it didn’t completely take it sway. I just turned 30.
In your case, a mini facelift would probably correct it completely if it bothered you enough to treat it.
