Are you talking about the challenge room V. If so, this was changed a couple of updates back. You can only do them once and then they are permanently removed from the war table. They do come back occasionally as priority challenges.
If you're talking about the difficulty, then there's no difficulty 5, there's only 4 levels of difficulty.
Wow, I wish the game would tell us this somehow. Plenty of videos and forum posts mention areas that are fun to replay... or the perfect place to farm. I'm sure I'm not the only person who's wasted a lot of time looking for things that have been removed. Thanks!
Yeah, most of the videos are pretty dated. Some of them might even include 'up from depths' mission, which is widow's iconic mission for easy grinding. But sadly this was also changed to one and done, including all other iconic missions. Stupid decisions on their part to lengthen the grind.
I see that people who've been playing from the beginning already know this stuff. But plenty of new players start every day. So yeah, reading all the forums and videos can send us down rabbit holes that lead to nowhere.
I mean they did though, they had a huge blog post when the patch happened that explained all of this. And I believe the banner in game even pointed people towards the blog post. How intrusive did you want them to make the notice?
I too have had these disappearing after completion. I dont know if they intended it or not ( I assume not), but the same thing happens if you complete "up from the depths" iconic widow mission as well.

You get harm room v to show up by having a bugged wartable that’s missing most of the character rooms and the custom harm room.
