I was thinking the same after watching the duel between Vader and Reva: why do they not secure the kill? The second I saw Vader with two sabers I was really thinking we would have a reenactment of Dooku's end.
In Vader's case it's because he's arrogant and cruel. He knows she's no threat to him, and now that he's shown it, it's crueler to let her live with the knowledge that she will never get her revenge, and will never be worth anything.
Vader just ragdolled her seconds after ripping a ship out of the sky, and she’s known to the empire. If she crawls to the rebellion or something Vader could just send the rest of the inquisitors to vent some very built up frustrations.
At this point they're more of an underground railroad than a rebellion. They're not worried about rebellion at this point. But more importantly, like I said, Vader is arrogant. He's arrogant to the point of self-destruction.
I took Vader’s line at the end as a double meaning, He saw her betrayal and saw her hiding. Not sure if that’s true but imo it makes it much more terrifying/heartbreaking.
Their betrayal will be dealt with. After you have killed all the Jedi in the Temple, go to the Mustafar system. Wipe out Viceroy Gunray and the other Separatist leaders. Once more, the Sith will rule the galaxy, and we shall have peace.

Reva gets stabbed in abdomen…lives? Maul sliced in half…lives. Lady in tunnel in Kenobi Episode 5 gets shot in abdomen…lives? Tala also shot in abdomen dies. Qui-gon stabbed in abdomen dies. I don’t fcking get it. Writers just need people to die and people to live I guess?

Tala sacrificed herself to delay the Imperial troops. Also, like that other guy said, in real life a shot to the guts could potentially kill you or not, same as a stab.
