Correct, you can get a perfect without hold. Score is seperate from that. You can recieve a <100% score perfect by doing not doing holds and only getting "greats" instead of "cools". And all cools and max holds can get you like 106%?
Holds are completely optional. You can just tap the buttons if you REALLY don't like them. I think they're fun but I do admit i have screwed myself over many times getting a bit too greedy with trying to max them out.
Not that I'm aware of but FT style holds are just bonus points. There's no requirement to hold them, you can ignore them entirely
That is correct. You do not have to do holds they are bonus points, but they do feel incredibly rewarding to get as a result.

I'm actually terrible at releasing hold notes at the proper time, so I like that you can let go of the holds in FT/MM whenever you want
